目的 探讨体检人群中芯片法多肿瘤标志物筛查的临床意义。方法 2015年3月至2015年9月大连医科大学附属第二医院体检中心健康体检者11156名。采集受检者静脉血清进行蛋白芯片法多肿瘤标志物的检测,检测项目为AFP,CEA,CA12-5,CA15-3,CA19-9,CA24-2,NSE,PSA,Free-PSA,β-HCG,GH,Ferritin 12项常见肿瘤标志物。选用CobasE411电化学发光仪对于阳性检测结果作为金标准进行单个项目肿瘤标志物检测复查,比较两种检测方法的结果。对于阳性检测结果者召回做病理学和影像学等进一步检查以明确诊断。结果 11156名体检者中蛋白芯片法检测出肿瘤标志物阳性者有246名,阳性率为2.20%。单个肿瘤标志物CobasE411电化学发光仪复查结果阳性233例,真阳性率为94.72%。其中33例经病理学、影像学等检查确定为早期或中期肿瘤,肿瘤总检出率为0.29%。结论 体检人群中蛋白芯片法多肿瘤标志物筛查有一定的阳性率,肿瘤标志物阳性的高危人群应定期体检监测相关指标并咨询专业临床医师,做到早发现早治疗。 |

Objective To discuss the clinical significance of multi-tumor markers protein chip screening during regular check-up in healthy population . Methods Totally, 11156 healthy volunteers were selected in the Medical center of Second Affiliated Hospital of Dalian Medical University from March,2015 to September 2015, including 5649 men and 5507 women. Venous serum samples of the volunteers were collected and centrifuged for multi-tumor markers Protein chip examination (including AFP , CEA , CA12-5 , CA15-3 , CA19-9 , CA24-2 , NSE , PSA , Free-PSA , β-HCG , GH , Ferritin ). Positive results of protein chip would be rechecked by single tumor marker technology, which was used as the golden standard. The highly suspected cases were recalled for further pathological examination and imaging studies to confirm the diagnosis. Results Among 11156 blood samples, 246 cases were positive for tumor marker according to protein chip technology with a positive rate of 2.20%. Among the 246 positive cases, 233 cases were positive after single standard technology with a true positive rate of 94.72%. Among them, 33 cases were diagnosed with early cancer by further examination of pathology and imaging studies. CEA had the highest proportion of positive indicators with a positive rate of 0.255% . Conclusion Multi-tumor markers protein chip screening in healthy population dose have certain positive rate. The tumor markers positive high-risk groups should receive regular medical monitoring for relevant indicators and consult a professional clinician to get early detection and early treatment. |
