hMSH2、MGMT在子宫内膜癌中的表达及其病理意义

摘要:
[目的]了解子宫内膜癌DNA修复蛋白hMSH2、MGMT的表达状况及其病理意义。[方法]应用免疫组织化学方法检测66例子宫内膜癌及24例正常子宫内膜组织hMSH2蛋白、MGMT蛋白的表达,并分析其与临床病理参数及P53蛋白表达关系。[结果]hMSH2、MGMT 和P53在子宫内膜癌的阳性表达率均高于正常子宫内膜组织,差异有显著性意义(P<0.05)。P53蛋白的表达率在子宫内膜癌Ⅰ~Ⅱ级组与Ⅲ级组分别为35.2%与83.3%,二者差异也有显著性意义(P<0.05);在淋巴转移组为61.5%高于无淋巴转移组的39.6%,但差异无显著性意义(P>0.05);在浅肌层浸润组与深肌层浸润组分别为34.0%与75%,二者差异有显著性意义(P<0.05)。在肿瘤组织中,hMSH2蛋白表达与P53表达呈正相关,但MGMT蛋白表达与P53无明显相关关系。[结论]P53蛋白的表达对子宫内膜癌的预后有一定预测意义。子宫内膜癌发生过程中p53突变可能与DNA复制过程中发生的碱基错配相关。

Abstract:
[Objective]To explore the expressions and pathological significances of DNA repair proteins hMSH2 and MGMT in endometrial carcinoma.[Methods]The expressions of hMSH2,MGMT and P53 protein were detected in 66 cases of endometrial carcinoma (EC) and 24 cases of normal endometrium (NE) by immunohistochemistry.[Result]All of P53,hMSH2 and MGMT positive expression rates in EC were significantly higher than those of NE (P<0.05).The positive expression rate of P53 inⅠ-Ⅱ pathologic stage group of EC (35.2%) was,significantly lower than that in Ⅲ pathologic stage (83.3%),and that in shallow muscle layer infiltration group (34%) was significantly lower than that in deep muscle layer infiltration group (75%) (P<0.05),and the rates in lymphatic node metastasis group and without were 61.5% and 39.6% respectively,while the difference was not significant (P>0.05).In EC,the expression of P53 was positively related to that of hMSH2 significantly,but not to that of MGMT.[Conclusion]The detection of P53 expression may help us to predict prognosis of the patients with endometrial carcinoma and mutation of P53 may be ralated to the mismatch of DNA in the development of endometrial carcinoma.
