复合保温对腹腔镜胆囊切除术患者体温及血流动力学的影响

摘要:
[目的]探讨在腹腔镜胆囊切除术中复合保温对患者体温及血流动力学的影响。[方法]将40例拟行腹腔镜胆囊切除术(LC)患者,随机分为复合保温组(A组,20例)和常规保温组(B组,20例),观察患者术中体温及血流动力学的变化。[结果]B组体温在气腹后10 min、术毕、术后3 h明显低于A组(P<0.01),B组的心率、收缩压、舒张压在气腹后10 min、术毕明显高于A组(P<0.05)。[结论]复合保温方法可有效防止患者手术中低体温的发生,并保持其血流动力学稳定,是一种安全、无刺激、无不良反应的保温方法。

Abstract:
[Objective]Heat preservation during operation has always been a challenge to anesthesiologist and nurse.We studied the effects of the comprehensive heat preservation methods on body temperature and hemodynamics of patients undergoing laparoscopic cholecystectomy surgery.[Methods]Forty patients,scheduled for selective laparoscopic cholecystectomy surgery,were randomly divided into two groups:20 patients in group A warmed with the comprehensive heat preservation methods during operation; 20 patients in group B warmed with the routine heat preservation methods during operation.The central and peripheral temperatures were monitored and hemodynamics parameters were recorded at different operation time points.[Results]Compared with group A,the body temperature of patients in group B was significantly lower at CO2 pneumoperitoneum 10 min,surgery ending and postoperative 3 h (P<0.01); SBP,DBP and HR of patients in group A were significantly increased at the pneumoperitoneum 10 min and surgery ending (P<0.05).[Conclusion]Comprehensive warming methods are effective in preventing in-operative hypothermia,and in keeping stable hemodynamics.It is a safe,non-irrateting and no side effects method.
