微量泵输注3%盐酸氯普鲁卡因在硬膜外麻醉中的临床应用

摘要:
[目的] 观察比较3%盐酸氯普鲁卡因(可普诺)在硬膜外麻醉中分次给药和微量泵持续给药效果的区别。[方法] 选择单纯硬膜外麻醉患者100例,随机分成两组,记录麻醉起效后病人的平均动脉压、阻滞范围变化和心率变化,以及肌松欠佳和镇痛欠佳的例数。[结果] 微量泵持续给药方式与分次给药方式相比对病人的影响更小,麻醉效果更佳,差异有显著性意义:微量泵持续输注3%盐酸氯普鲁卡因组(微量泵组)的最大阻滞范围明显小于对照组,差异有显著性意义(P<0.01);术中平均动脉压下降幅度与对照组相比明显减少,差异有显著性意义(P<0.01);麻醉以后微量泵组)心率增加明显小于对照组,差异有显著性意义(P<0.01),肌松效果欠佳和辅助镇痛例数(微量泵组)组明显少于对照组(P<0.01)。[结论] 应用微量泵持续性给药能够取得完善的麻醉效果,对病人干扰更小,更安全。

Abstract:
[Objective] To compare the efficacies of intermittent infusion and continuous infusion of 3% chlorprocaine hydrochloride with micro-pump in epidural anesthesia. [Methods] One hundred patients were randomly divided into two groups, the maximum block range, MAP, HR, The number of cases of muscle relaxant effects of poor and supporting analgesia were recorded. [Results] The effect of continuous infusion with micro-pump was better than intermittent infusion and significant differences in analgesia effects were observed. The maximum block range of micro-pump group was significantly less than that of the control group (P<0.01). The MAP of micro-pump group decreased during the anesthesia was significantly less than that in the control group (P<0.01). The heart rate increased was significantly less than that in the control group (P<0.01). The number of cases of muscle relaxant effects of poor and supporting analgesia was significantly less than that in the control group (P<0.01). [Conclusion] Continuous infusion of 3% chlorprocaine hydrochloride with micro-infusion pump in epidural anesthesia is perfect and safe.
