大肠癌P21、CDK4蛋白表达的相关性和临床意义

摘要:

［目的］ 探讨大肠癌P21、CDK4蛋白表达的临床意义。［方法］采用SP法检测P21、CDK4在正常肠黏膜组织、大肠腺瘤和大肠癌中的表达情况。［结果］P21阳性表达率在正常黏膜组织、腺瘤与大肠癌中为88.9%、66.7%与40.8%,肿瘤组织明显低于正常黏膜（P<0.05）；在大肠癌高、中与低分化3组中分别为50.0%、52.2%与14.3%；Duke A+B期（52.4%）明显高于C+D期(32.1%)；淋巴结转移阴性组(58.8%)明显高于阳性组(31.3%)（P<0.05 ）。CDK4阳性表达率在正常黏膜、腺瘤与大肠癌中为11.1%、33.3%与57.1%，呈递增趋势（P<0.05 ）；大肠癌高、中与低分化3组分别为25.0%、56.5%与85.7%；Duke A+B期(38.1%)明显低于C+D期(71.4%)；淋巴结转移阴性组(52.9%)与阳性组(59.4%)差异不明显。［结论］P21表达减少或缺失和CDK4的表达上调，可作为判定大肠癌恶性程度及预后的有效指标。

Abstract:

［Objective］ To explore the role of expression of P21 and CDK4 in diagnosing colorectal carcinomas.［Methods］ The expression of P21 and CDK4 of normal intestinal mucosas,colorectal adenomas and human colorectal carcinomas were examined by immunohistochemistry. ［Results］The positive rates of P21 expression in normal intestinal mucosa,colorectal adenmoa and colorectal carcinomas were 88.9%,66.7% and 40.8%, respectively(P<0.05).The rates in well,moderately and poorly differentiated carcinomas were 50.0%,52.2% and 14.3%,respectively.The rate in Duke A＋B phase(52.4%) were more than C＋D phase(32.1%).The rate of the group without lymph node metastasis(58.8%) was higher than that of the group with lymph node metastasis(31.3%).The expression of CDK4 protein in normal intestinal mucosa,colorectal adenoma and colotectal cardinomas were 11.1%,33.3% and 57.1%, respectively (P<0.05). The rates in well,moderately and poorly differentiated carcinomas were 25.0%, 56.5% and 85.7%,respectively.The rate of Duke A＋B phase (38.1%) was lower than C＋D phase (71.4%) (P<0.05). To compare the rate of the group without lymph node metastasis(52.9%)with that of the group with lymph node metastasis(59.4%) is not respective(P＞0.05).［Conclusion］The low expression of P21 protein and the high expression of CDK4 are the significance index to assessment the malignant level of colorectal carcinoma prognosis.
