胃粘膜PCNA、Fas表达与Hp感染及癌变关系的研究

摘要:

探讨胃上皮细胞增殖、凋亡与幽门螺杆菌(Hp)感染的关系及Hp感染致癌的可能机制。［方法］　应用快速尿素酶试验与W-S银染色法将各种病变分为Hp感染阳性组和阴性组，应用免疫组化SP法对各种病变增殖细胞核抗原（PCNA）、凋亡相关蛋白（Fas）的表达进行检测。［结果］Hp阳性组PCNA表达明显高于Hp阴性组（P<0.005）,Fas表达随病变程度的加重而逐渐加重，但只有在浅表性胃炎组中Hp阳性者Fas表达高于Hp阴性者(P<0.02)。［结论］Hp感染可促进胃上皮细胞PCNA及Fas表达增加，且Hp的这种作用主要发生在胃粘膜病变的早期。Hp促进增殖和凋亡可能是其致癌的重要机制之一。

Abstract:

To observe the relationship between Hp infection and proliferation as well as apoptosis of gastric epithelial cells and further elucidate the possible effect of Hp infection on gastric carcinoma pathogenesis. ［Methods］　The expressions of PCNA and Fas were examined by immunohistochemical SP technique in gastric epithelial cells, and divided into four grades (-,+,++,+++) according to percentage and staining intensity of positive cell. ［Results］ Of all patients, the expression of PCNA in the Hp positive group were significantly higher than that in the Hp negative group (P<0.005). The expression of Fas was increased from CSG, CAG, IM to Dys, and reached a peak in GCa. But only in CSG was the expression of Fas higher in Hp positive group than in Hp negative group(P<0.02). The expression of Fas in Hp positive CSG was as same as in Hp positive CAG(P>0.20), while the expression in Hp negative CSG was lower than in Hp negative CAG(P≤0.05). ［Conclusion］ Increased gastric epithelial cell proliferation and apoptosis associated with Hp infection, which were seen mainly in the initiating stages in gastric musocal lesions is one of the pathegenesis of gastric carcinoma
