目的 探讨青年乳腺癌与中年、老年乳腺癌在超声影像学、病理及免疫组化表现方面的差异。方法 共收集乳腺癌患者162例,按年龄分为青年组(≤40岁)、中年组(41~59岁)和老年组(≥60岁) 各54例。回顾分析三组乳腺癌在超声影像学、病理及免疫组化表现方面的差异。结果 青年组超声恶性征象“边缘毛刺征”、“周围高回声晕环”、“微小钙化灶”、“后发回声衰减”、“腋窝异常肿大淋巴结”的比率均高于中年组和老年组(P<0.05)。青年组浸润性非特殊性癌III级、肿瘤大小T3+T4、腋下淋巴结转移的比率均高于中年组和老年组(P<0.05)。青年组ER、PR阳性表达比率均低于中年组和老年组(P<0.05)。结论 青年乳腺癌与中老年乳腺癌相比具有恶性度高、侵袭性强及超声典型恶性征象发生率高的特点。 |

Objective To evaluate the differences of ultrasonographic, pathologic and immunohistochemical features of breast carcinomas among young, middle-aged and elderly women.Methods A total of 162 patients with breast cancer were enrolled. According to age, the 162 patients were divided into three groups, including youth-age group (≤40 years old, 54 cases), middle-age group (41-59 years old, 54 cases), and old-age group (≥60 years old, 54 cases). Retrospective analysis was performed to evaluate the differences of ultrasonographic, pathologic and immunohistochemical features among the three groups. Results The ratios of grade Ⅲ infiltrating non-specific carcinoma, tumor size T3+T4, axillary lymph node metastasis in the youth-age group were larger than those in the middle-age group and the old-age group(all P<0.05). The ER and PR positive rates in the youth-age group were lower than those in the middle-age group and the old-age group (all P<0.05);Compared with the middle-age group and the old-age group, the youth-age group more commonly presented with manginal apicule sign, hyperechoic halo, microcalcification, posterior shadowing and axillary lymph node enlargement on ultrasonographic imaging(all P<0.05).Conclusion Breast carcinomas of young women are more commonly presented with higher degree of malignancy, stronger invasiveness and typical malignant ultrasonographic features. |
