目的 探讨原发性高血压(EH)患者同型半胱氨酸(Hcy)和空腹血糖(FPG)水平与颈动脉粥样硬化(CAS)的相关性。方法 采用酶法和已糖激酶法分别检测88例EH患者的血清Hcy和FPG水平。根据颈动脉内膜中层厚度(CIMT)分为≤0 mm组(CIMT非增厚组,30例)、内膜中层厚度>0 mm组(CIMT增厚组,58例)。以50名健康体检者作为正常对照组。检测各组Hcy(酶法)、FPG(已糖酶法)以及血液中的总胆固醇(TCH)(酶法)、甘油三酯(TG)(酶法)、低密度脂蛋白(LDL-C)(直接法)、高密度脂蛋白(HDL-C)(直接法)值,并分析Hcy、FPG、TCH、TG、LDL-C、HDL-C与CIMT的相关性。结果 CIMT非增厚组与对照组相比Hcy和FPG水平升高(P<0.05)。CIMT增厚组与CIMT非增厚组相比Hcy和FPG水平升高(P<0.05),与对照组相比Hcy和FPG水平升高(P<0.01)。CIMT增厚组CIMT与Hcy和FPG呈正相关(r=0.439和r=0.537,P<0.05)。CIMT非增厚组CIMT与Hcy和FPG呈正相关(r=0.545和r=0.760,P<0.05)。对照组CIMT与Hcy和FPG也呈正相关(r=0.416和r=0.526,P<0.05)。Logistic回归分析表明Hcy和FPG均是EH患者CAS独立危险因素。结论 Hcy和FPG水平与EH合并CAS具有相关性,且为EH患者CAS的独立危险因素。 |

Objective To investigate the correlation of homocysteine(Hcy) and fasting plasma glucose(FPG) levels with carotid atherosclerosis(CAS) in essential hypertension patients(EH).Methods Hcy and FPG levels in 88 patients with EH were determined by enzymic method and sugar kinase method.According to carotid intima-media thickness,138 patients with EH were classified into CIMT thickening group (58 cases,CIMT>0 mm) and CIMT non-thickening group (30 cases,CIMT≤0 mm).A total of0 healthy subjects were enrolled as a control group.Hcy(enzymic method),FPG(have sugar kinase method),TCH(enzymic method),TG (enzymic method),HDL-C (the direct method) and LDL-C (the direct method) were determined in each group.Correlations of Hcy,FPG,TCH,TG,HDL-C and LDL-C were analyzed.Results Hcy and FPG levers in non-thickening group were higher than those in control group (P<0.05).Hcy and FPG levers in thickening group were higher than those in non-thickening group(P<0.05).Hcy and FPG levers in thickening group were higher than those in control group (P<0.01).FPG and Hcy levers in thickening group were positively correlated with CIMT(r=0.439 and 0.537,P<0.05).Hcy and FPG levers in non-thickening group were positively correlated with CIMT(r=0.545 and 0.760,P<0.05).FPG and Hcy levers in control group were positively correlated with CIMT(r=0.416 and 0.526,P<0.05).Logistic multivariate regression analysis was performed to analyze the risk factors of CAS.Hcy and FPG levers were one of the independent risk factors of CAS in EH.Conclusions Hcy and FPG levers are correlated to EH combined with CAS.The combined detection of Hcy and FPG levers contributes to the condition and prognosis judgement.Hcy and FPG levers are one of the independent risk factors of CAS in EH. |
