甲状腺全切除术与近全切除术治疗分化型甲状腺癌的效果分析

摘要:

[摘要]　目的　探究甲状腺全切除术与近全切除术治疗分化型甲状腺癌的临床效果。　方法　选取 2011年1月—2014年1月行手术治疗的分化型甲状腺癌病人126例，分为全切组61例和近全切组65例，比较两组病人术后并发症发生情况、生存情况、肿瘤复发和转移情况等。　结果　甲状腺全切除组病人术后呼吸困难、声音嘶哑和低钙血症等并发症的发生率高于近全切除组病人，差异具有显著性意义(P<0.05)。喉上神经损伤在甲状腺全切除组发生率高于近全切除组，但差异无显著性意义(P>0.05)。两组病人在随访期间均未出现复发和淋巴结转移。　结论　甲状腺近全切除术治疗甲状腺癌有较好的临床效果，与甲状腺全切除术相比术后并发症少，预后无明显差异，值得临床推广。

Abstract:

[Abstract]　Objective　To explore clinical effect of total thyroidectomy and subtotal thyroidectomy in the treatment of differentiated thyroid carcinoma. 　Methods　We selected 126 patients, who were diagnosed with differentiated thyroid cancer and received surgical treatment in our hospital from 2011 January to 2014 January. The patients were divided into total resection group (61 cases) and subtotal resection group (65 cases). Postoperative complications, survival, recurrence and metastasis of the tumor were compared between the two groups. 　Results　The rates of dyspnea, hoarseness and hypocalcemia were higher in total resection group than those in subtotal resection group (P<0.05). The superior laryngeal nerve damage in patients of total resection group were higher than those in subtotal resection group patients, there was no significant difference (P>0.05). There was no recurrence and lymph node metastasis in two groups of patients during follow-up. 　Conclusion　Subtotal thyroidectomy for thyroid cancer has good clinical effect, less postoperative complications and similar prognosis compared with total thyroidectomy. It is worth to promote in clinic.
