MRI在滑膜软骨瘤病中的诊断价值

摘要:
[目的]探讨MRI在滑膜软骨瘤病中的临床诊断价值。[方法]回顾分析经病理证实的15例滑膜软骨瘤病患者的X线及MRI影像资料,并与Milgram病理分期对照,其中膝关节7例,髋关节5例,肘关节、肩关节及踝关节各1例。[结果]滑膜软骨瘤病的MRI表现为滑膜增厚、关节内游离体、关节积液和关节退行性改变。不同时期的滑膜软骨瘤病的MRI表现不尽相同,Milgram Ⅰ期患者X线平片阴性,MRI显示滑膜的结节状增厚和软骨化生倾向;MilgramⅡ、Ⅲ期的患者除滑膜增厚外,关节内游离体表现为软骨结节和骨性结节,结节表面可有强化。[结论]MRI检查可以提供更多关节内游离体和滑膜病变的信息,对于滑膜软骨瘤病的诊断和分期提供帮助,特别是对X线检查阴性的Milgram Ⅰ期病例具有重要价值。

Abstract:
[Objective]To investigate the clinical value of magnetic resonance imaging (MRI) in synovial chondromatosis.[Methods]Fifteen patients with histologically confirmed synovial chondromatosis underwent imaging and X ray examinations.Among these patients,there were 7 knee joints,5 hip joints,1 elbow joint,1 shoulder joint and 1 ankle joint.[Results]MRI clearly showed thickened synovium,free bodies,joint effusion and joint degenerative lesions.The MRI manifestation of synovial chondromatosis varied with stages.For the Milgram-I stage cases,without positive features in plain film.MRI was superior to X-ray in showing the number and position of free body and synovial lesions in the cases of cartilaginous.[Conclusion]MRI provides much more information about free body and synovial lesions,which is useful especially in the diagnosis of Milgram-I stage cases that is negative in X-ray examination.
