310例未通过听力筛查新生儿听力诊断

摘要:
[目的]探讨未通过听力筛查新生儿听力诊断方法。[方法] 用短音诱发脑干听觉诱发电位(click-auditory brainstem responses,click-ABR)和多频稳态听觉诱发电位(auditory steady-state responses,ASSR)及鼓室声导抗技术对听力筛查未通过新生儿进行听力学评估,均在出生6个月内明确诊断,并进行听力学跟踪随访至1~2岁。[结果]310例听力筛查未通过者于出生6个月内诊断为听力正常者260例,占未通过听力筛查的83.9%;click-ABR听力评估结果异常43例,占未通过听力筛查的13.9%;click-ABR听力评估结果正常,ASSR检测异常者7例,占未通过听力筛查的2.3%;鼓室声导抗测试异常10例,占未通过听力筛查的3.2%。[结论]对未通过听力筛查新生儿需进行click-ABR和ASSR及鼓室声导抗检测及动态随诊观察,从而给出准确的诊断。

Abstract:
[Objective]To investigate the hearing diagnostic method for newborns failing in the hearing screening. [Methods]Click-auditory brainstem responses (Click-ABR), auditory steady-state responses (ASSR )and tympanometry were used to test newborns failing in the hearing screening from 40 days to 6 months, then we follow up them until 1~2 years old. [Results] Two hundred and sixty infants were diagnosed with normal hearing in 6 months old, the rate was 83.9% of 310 infants who failed hearing screening; 43 infants were diagnosed with abnormal hearing by Click-ABR test (13.9%); 7 infants were diagnosed with normal results by Click-ABR test while abnormal results by ASSR test(2.3%); 10 infants were diagnosed with abnormal results by tympanometric test (3.2%). [Conclusion]In order to make an accurate diagnosis, protect infants from dysphasia, it is necessary for infants failing in the hearing screening to have tests of click-ABR, ASSR, tympanometry and be followed up until 1~2 years old.
