血管反应性检测对缺血性脑卒中的诊断价值

摘要:
[目的]探讨血管反应性对缺血性脑卒中的诊断意义。[方法]对85例门诊及住院的缺血性脑血管病患者及60例非脑血管病患者行双功能彩色多普勒检测。评价指标包括颈动脉斑块、颈动脉狭窄以及血管反应性(Vasoreactivity)。将颈动脉狭窄患者的血管反应性与对照组的血管反应性进行分组统计。统计学处理以P<0.05为具有统计学意义。[结果]①85例缺血性脑血管病患者中轻度狭窄29例,中度狭窄8例,重度狭窄5例,颈动脉闭塞3例,其余40例无颈动脉狭窄。对照组中轻度狭窄5例;中度狭窄2例;重度狭窄0例。②缺血性脑血管病颈动脉狭窄组与对照组非狭窄组的血管反应性差异有显著性意义,P<0.05。[结论]①颈动脉狭窄患者的血管反应性明显下降。②彩色多普勒显示颈动脉效果良好,可作为颈动脉狭窄的筛选工具。

Abstract:
[Objective]To explore the diagnostic value of duplex color doppler ultrasonography(DCDU) in carotid stenosis.[Methods]DCDU examination was performed in 85 patients(including outpatients and inpatients) with ischemic cerebrovascular disease (ICVD) and in 60 healthy controls. The evaluating indexes included carotid atherosclerotic plaque, carotid stenosis as well as vasoreactivity. Based on the results of DCDU examination, the patients and the controls were classified and statisticsed according to the severity of stenosis, respectively. The vasoreactivity in ICVD patients having carotid stenosis was compared with that in the normal controls. P<0.05 was considered as statistically significant.[Results]① Of all 85 patients with ICVD, 29 had mild stenosis, 8 had moderate stenosis, 5 had severe stenosis, 3 had complete occlusion, and the other 40 showed no stenosis of the carotid artery. Of 60 controls, only 5 had mild stenosis, 2 had moderate stenosis, whilie nobody had either severe stenosis or complete occlusion of the carotid artery. The vasoreactivity in ICVD patients having carotid stenosis was significantly lower than that in the controls without carotid stenosis (P<0.05).[Conclusion]① The vasoreactivity in patients with carotid stenosis decreases remarkably. ② DCDU can be used as a screening tool to detect carotid stenosis due to its safety and effectivity.
