MSCT及重建技术在小肠疾病诊断中的应用价值

摘要:
[目的]探讨MSCT小肠重建技术及其在小肠疾病诊疗中的临床应用价值。[方法]对20例怀疑小肠疾病病人进行了口服稀释造影剂法的MSCT成像,并与全消化道造影、手术病理对照分析。采用最新的GE Light-speed Ultra 16排CT进行全小肠扫描,采用仰卧位,扫描条件:扫描层厚1.25 mm,螺距1.375∶1,转速0.8 s。在独立的工作站上进行小肠重建,包括MPR像、MIP像。图像由两位影像医师共同分析。[结果]20例均经临床和/或病理证实,其中正常7例,良性病变4例,恶性病变9例。 MSCT多种成像方法均可提出有价值的倾向性意见,其中MPR像和MIP像价值最大,可以完整直观观察病变的部位、特点以及距离等信息。小肠CT检查基本是以口服稀释造影剂为主,尤其是对于肠内的实质性占位,轴位像结合MPR和MIP像是主要的成像方法。2D-MIP像有助于末段肠管病变距离的测定。增强检查对于小肠病变中整体显示均有帮助。[结论]多层CT小肠成像技术对于小肠疾病的术前准确定位、定性以及鉴别诊断具有较高的临床应用价值。

Abstract:
[Objective]The purpose of this study is to discuss small intestine imaging technique with multi-slice CT and its clinical application value in diagnosis and heal in small intestine diseases.[Methods]Twenty cases of patient with clinically suspicious small intestine diseases were performed multi-slice CT imaging, proved clinically or pathologically. Using GE Light-speed Ultra 16 slice CT to perform whole small intestine scan in the supine. Scanning condition: 1.25 mm/ 1.375∶1/0.8 s.Three-dimensional images of small intestine were done at the console workstation, including MPR images and MIP images. Two imaging doctors together analyze CT images.[Results]Normal, benign and malignant lesions are respectively 7 cases, 4 cases and 9 cases. CT imaging may bring forward valuable orientation opinion without exception. Among these, MPR images and MIP images were best and helpful to operation and diagnosis. Drink water was main small intestine imaging technique with multi-slice CT basically, especially to substantiality tumor in small intestine, and axis images combined with MPR, MIP images were main imaging means.[Conclusions]Small intestine examination with multi-slice CT is very valuable to exact pre-operational orientation in patients with small intestine diseases and differentiated diagnosis.
