rhEGF在外伤性鼓膜穿孔治疗过程中的应用

摘要:
[目的]研究重组人表皮生长因子(recombinant human epidermal growth factor, rhEGF)在治疗外伤性鼓膜穿孔过程中的作用。[方法] 对门诊就诊的外伤性鼓膜穿孔患者123例患者,随机的分别采用4种不同的治疗方法(即常规治疗方法、贴膜法、应用rhEGF喷剂法、贴膜和rhEGF喷剂联合应用法),定期观察鼓膜愈合情况、瘢痕的轻重,记录鼓膜完全愈合的时间,按鼓膜穿孔的大、中、小,分3组进行统计学处理。[结果]对小穿孔4种治疗方法差异无显著性意义(愈合时间,P>0.05),对中等穿孔,传统组、贴膜组与rhEGF组、贴膜+rhEGF组之间分别两两比较差异有显著性意义(P<0.05)。对大穿孔,传统组、贴膜组、贴膜+ rhEGF组三者之间两两比较差异均有显著性意义(P<0.05)。[结论]外伤性鼓膜穿孔中的中等穿孔和大穿孔应用rhEGF或rhEGF与贴膜联合应用,可以显著地缩短愈合时间,并减轻鼓膜瘢痕,提高治愈率。

Abstract:
[Objective]To evaluate the application of rhEGF (Recombinant Human Epidermal Growth Factor) in the treatment of traumatic tympanic membrane perforation.[Methods]One hundred and twenty-three outpatients of traumatic tympanic membrane perforation were divided into four groups randomly (i.e. A: routine therapy, B: membrane-sticking, C: spray rhEGF only, D: combination of membrane-sticking and spray rhEGF). Following up includes the healing condition of tympanic membrane, the degree of scar and the duration of complete healing. The cases were also divided into three groups (large, medium and small) according to the size of the perforation in order to be calculated statistically.[Results]There was no significant difference among the four kinds of therapy in the treatment of small perforations (the healing duration, P< 0.05). There was significant difference between group A and group B, also between group C and group D in the treatment of medium perforations (P<0.05). In the management of large perforation, there was significant difference among the groups of A, B and D (P<0.05).[Conclusions]The utility of rhEGF only or combination with membrane-sticking is able to shorten the healing duration significantly and alleviate the scar in the treatment of medium and large traumatic perforation of tympanic membrane.
