地塞米松对阿曲库铵神经阻滞效应的影响

摘要:
[目的]观察地塞米松对阿曲库铵神经肌肉阻滞效应的影响。[方法] 选择择期甲状腺肿物切除术病人80例,随机分成两组:地塞米松组,麻醉诱导前静脉注射地塞米松10 mg,10 min后按顺序静脉注射咪唑安定0.1 mg/kg、芬太尼 4 μg/kg、丙泊酚2 mg/kg、阿曲库铵0.6 mg/kg;对照组,麻醉诱导前给予同等剂量生理盐水,之后给予相同剂量麻醉诱导药物。观测两组患者拇内收肌群TOF,记录T1消失的时间及第1次T0F恢复时T1出现的时间。[结果] 地塞米松组阿曲库铵起效时间为(289.0±19.7) s,比对照组(226.4±11.2) s明显延长,恢复时间为(13.7±0.5) min,比对照组(20.8±3.2) min明显缩短(P<0.05)。[结论]地塞米松可减弱阿曲库铵对全麻病人的神经肌肉阻滞效应。

Abstract:
[Objective]To observe the effects of dexamethasone on neuromuscular blocking efficiency of atracurium. [Methods]Eighty patients with ASA I-II who underwent thyroid surgery were randomly divided into two groups : dexamethasone group, the patients received intravenously 10 mg dexamethasone before anesthesia induction, then were given midazolam 0.1 mg/kg, fentanyl 4 μg/kg, propofol 2 mg/kg, atracurium 0.6 mg/kg;control group, the patients were given same volume saline before anesthesia induction,then anesthesia induction were taken with same drugs and dosage. Train-of-four(TOF)were applied to ulnar nerve-adductor pollicis,time of T1 disappeared and recovered at first time was recorded.[Results]Time of onset of atracurium of Dexamethasone group (289.0±19.7)s prolonged significantly than that of control group (226.4±11.2)s (P<0.05); and recovery time of atracurium of Dexamethasone group (13.7±0.5)min was shorter significantly than that of control group (20.8±3.2)min (P<0.05). [Conclusion]Dexamethasone may attenuate the efficiency of neuromascular blockage of atracurium.
