单核细胞趋化蛋白-1和核因子-κB与巨噬细胞浸润及动脉粥样硬化斑块形成的关系

摘要:
[目的]]研究单核细胞趋化蛋白-1和核因子-κB与巨噬细胞浸润、冠状动脉粥样硬化斑块形成及冠状动脉进行性狭窄的关系。[方法]从39例尸检标本中获得39个冠状动脉左前降支粥样硬化的标本。应用弹力纤维染色及电脑软件分析系统,测量其狭窄程度,并根据其狭窄程度将其分为3组(A组:<50%;B组:50%~75%;C组:>75%)。B组与C组中又根据粥样斑块占内膜面积的百分比分别以20%和30%为界,分为两个亚组,即B1(<20%)、B2(≥20%)和C1(<30%)、C2(≥30%)。用免疫组织化学的方法来检测巨噬细胞(CD68),单核细胞趋化蛋白-1和核因子-κB在冠状动脉斑块中的表达情况。[结果]①B组与C组的中膜巨噬细胞浸润的个数(6.38±7.68,6.09±4.95)明显高于A组(0.78±0.67),P<0.05。B2和C2的斑块面积占内膜面积的百分比(42.94±6.54,67.20±15.63)和巨噬细胞数(189.17±78.92,195±71.93)明显高于B1(16.74±3.18,5.20±27.52)和C1(16.84±4.89,110±13.60),P<0.05。②MCP-1和NF-κB的阳性表达出现在粥样斑块处的内皮细胞、巨噬细胞及平滑肌细胞,尤以在泡沫细胞处更加明显;而且两者之间存在显著正相关(r=0.632,P<0.001)。[结论]]动脉局部核因子-κB和MCP-1的过度表达可能参与并调节局部炎症反应及巨噬细胞的浸润,而且与冠状动脉粥样硬化斑块形成及管腔进行性狭窄关系密切。

Abstract:
[Objective]To investigate the correlation between monocyte chemotactic protein-1, nuclear factor-kappa B and the macrophage infiltration into intima, then and the formation of coronary atherosclerotic plaque, the last and the progressive stenosis of coronary arteries. [Methods]The samples, thirty-nine left anterior descending coronary arteries, were gotten from thirty-nine autopsied cases. The stenosis degree was measured by elastic fibers straining and software analysis system. Then according to the degree, they were divided into 3 groups (A: < 50%; B: 50% ~75%; C: >75%). Based on the percentage of the plaque in intima, both B and C were divided into two sub-groups (B1<20%,B2≥20%;C1<30%,C2≥30%). The expression of macrophages(CD68), MCP-1 and NF-kB was examined by immunohistochemistry.[Results] ①The macrophages counting in media of B and C(6.38±7.68,6.09±4.95)were much more than that of A (0.78±0.67),P<0.05. The plaque area and macrophages counting of B2 and C2 (42.94±6.54,189.17±78.92; 67.20±15.63,195±71.93)were obviously larger than B1 and C1(16.74±3.18,5.20±27.52;16.84±4.89 ,110±13.60),P<0.05. ②MCP-1 and NF-κB expressed in endothelium cells, macrophages and smooth muscle cells, especially in foam cells. And there was positive relationship between them(r=0.632,P<0.001). [Conclusions]The excessive expression of MCP-1 and NF-kB in coronary atherosclerosis probably regulated the local inflammation and the macrophage infiltration, and was closely related to the formation of coronary atherosclerotic plaques and the progressive stenosis of coronary arteries.
