[目的] 探讨抗核抗体(ANA)、抗双链DNA(抗ds-DNA)抗体在自身免疫性疾病中的诊断价值, 找出能及时为临床提供快捷可靠检验数据的检测手段。 [方法] 用间接免疫荧光法检测ANA,用金标免疫斑点法检测抗ds-DNA,并以健康体检者作对照,以此评价滴度检测方法的临床应用价值。[结果] 检测227例系统性红斑狼疮(SLE)患者阳性率为100%,其中活动期病人75 例,滴度在1∶1000以上占92%,非活动期病人152例滴度在1∶100~1∶320之间的占98.7%;抗ds-DNA阳性率为89%,其中活动期病人滴度在1∶8以上的占25.3%,非活动期病人滴度在原倍~1:2之间的占80.3%;295例风湿性关节炎(RA)患者ANA阳性率为42.7%,抗ds-DNA阳性率为0.7%;339例急慢性肾炎患者ANA阳性率为29%,抗ds-DNA的阳性率为3%。 [结论] RA和肾病患者血清中ANA、抗ds-DNA均有阳性,但滴度较低,而SLE患者血清中ANA滴度较高,尤其是活动期时,两种抗体滴度的联合检测可显著提高SLE的诊断,及与其他自身免疫性疾病的鉴别诊断。 |

Objective] To study the diagnosis value of ANA and ds-DNA in all kinds of correlative diseases,and to find out the detection methods to apply quick and dependable test data for clinic.[Methods] ANA was detected by indirect immunological fluorescence method,and ds-DNA was detected by golden-marked immunological dot method while the normal healthy acted as control.The clinical application value were evaluated.[Results] The positive rates of 227 cases of SLE was 100%,among which there were 75 patients in active phase. 92% of 75 cases were 1∶1000 diluted positive. 98.7% of 152 in active phase were 1∶100~1∶320
diluted positive.The positive rate of ds-DNA was 89%, 25.3% in active phase were 1:80 diluted positive. 80.3% of patients in inactive phase were 1∶1~1∶2 diluted positive. The positive rates of ANA in 295 cases of RA were 42.7%. The positive rates of ds-DNA was 0.7% and the positive rate of ds-DNA was 3%.[Conclusion] ANA and ds-DNA in RA and renal diseases can be detected, but their concentration were low. ANA in SLE, especially in active SLE was positive in high concentration. And the cornbined detection of two antibodies can improve the diagnosis with other auto-immunological diseases. |
