全身炎性反应综合征时中性粒细胞凋亡的异常

摘要:
[目的]研究中性粒细胞即多形核白细胞(polymorphonuclear neutrophil,PMN)凋亡的异常在全身炎性反应综合征(systemic inflammatory response syndrome, SIRS)中的意义。[方法] SIRS患者8例(均为急性胰腺炎患者)为SIRS组,正常人6例为对照组。采用流式细胞仪和TUNEL方法定量检测外周血PMN的凋亡情况,采用ELISA法检测血清IL-6、IL-8的水平。[结果]流式细胞仪检测:SIRS组外周血PMN的细胞凋亡率为(42±8)%,低于对照组的(63±9)%,P<0.05; TUNEL法检测:SIRS组外周血PMN的凋亡指数(apoptosis index,AI)为25±7,低于对照组的37±10,P<0.005。SIRS组外周血血清IL-6水平为(8.2±2.2)pg/mL,高于对照组的(2.1±1.3)pg/mL,P<0.001;SIRS组外周血血清IL-8的水平为(38±12)pg/mL,高于对照组的(12±4)pg/mL,P<0.001。[结论]SIRS患者存在PMN的凋亡延迟,其在SIRS的发生、发展中有着重要的意义。

Abstract:
[Objective]To study the dysregulation of the polymorphonuclear neutrophil (PMN) apoptosis in the systemic inflammatory response syndrome (SIRS), and the clinical significance. [Methods] Eight acute pancreatitis patients with SIRS, 6 healthy control subjects were observed about the following: apoptosis of PMN in peripheral blood samples analysed by flow cytometry and TUNEL; the values of serum IL-6、IL-8 detected by ELISA. [Results] Spontaneous apoptosis was significantly delayed in PMN from patients with SIRS compared with controls (42±8)% vs (63±9)% by flow cytometry, P<0.05 and 25±7 vs 37±10 by TUNEL, P<0.005. The levels of IL-6 in the SIRS patients was higher than that of controls (8.2±2.2)pg/mL vs (2.1±1.3)pg/mL, P<0.001, the levels of IL-8 in the SIRS patients was higher than that of controls (38±12)pg/mLvs (12±4)pg/mL, P<0.001. [Conclusions] Circulating PMN from acute pancreatitis patients with SIRS show delayed apoptosis, and prolonged PMN survivalmay contribute to the systemic inflammatory injury characteristic of SIRS.
