糖抗原SLex、Lex、Lea在肺癌中的表达及与病人预后的关系

摘要:
研究Lewis糖抗原在肺癌及其淋巴结转移灶的表达与作用。[方法] 免疫组化LSAB法检测SLex、Lex、Lea在肺癌组织中的表达。[结果] 在70例非小细胞肺癌原发灶中,SLex、Lex、Lea的阳性率分别为72.9%(51/70)、54.3%(38/70)和44.3%(31/70),其淋巴结转移灶中,阳性率分别为67.1%(47/70)、47.1%(33/70)和40%(28/70);原发灶与转移灶SLex表达具有显著相关性。3种Lewis糖抗原在8例小细胞肺癌均无表达。鳞癌与腺癌低分化型Slex阳性率为85.7% (12/14)与92.3% (12/13),分别高于高分化者的60% (9/15)与65% (13/20);晚期病例高于早期病例。发生转移、复发或死亡的病例,SLex表达高于未发生者。[结论] Lewis抗原表达可能与肿瘤细胞组织来源和/或分化有关;SLex可成为肺癌病人预后的判断因素。

Abstract:
Objective] To investigate the relationship of expression of Lewis antigens in primary pulmonary carcinoma to lymph node metastasis. [Method] The expressions of SLex、Lex、Lea of lung cancer were detected by immunohistochemical technique in 70 cases of none small cell lung cancers(NSCLC). [Results] Expression rates of SLex、Lex and Lea in primary lesions were 72.9%(51/70)、54.3%(38/70) and 44.3%(31/70) respectively,and the metastasis lymph node lesions were 67.1%(47/70)、47.1%(33/70) and 40%(28/70) respectively; The expression of SLex in primary ones correlated to those of lymph node metastasis. The expression rates of SLex in poorly differentiated squamous cell carcinoma (85.7% ,12/14) and adeno-carcinoma (92.3%,12/13) were higher than those in well differentiated ones, separately 60% (9/15) and 65% (13/20). The expression rate of SLex in lung cancer with stage II was higher than that with stage I. The expression rate of SLex was higher than those of Lex and Lea. The expression rate of SLex in patients with bad prognosis was higher than that without. [Conclusion] The expressions of Lewis antigens correlate to the origin and differentiation, and infiltration and metastasis in NSCLC.
