肾癌伴腔静脉内瘤栓的外科治疗（附7例报告）

摘要:

探索肾癌伴腔静脉瘤栓形成病人的治疗及预后的情况。［方法］对7例肾癌合并腔静脉瘤栓病人进行根治性手术治疗，术后随访2年。［结果］7例病人通过手术治疗均完整切除肿瘤及腔静脉瘤栓。有肾门淋巴结转移者同时清除淋巴结。术后病理回报：4例为透明细胞癌，其他3例为少见病理类型。透明细胞癌4例病人中3例术后存活两年复查未发现有肿瘤复发，1例失访，其余3例均于术后1年内死于肿瘤复发。［结论］肾透明细胞癌伴腔静脉瘤栓的病人仍可施行根治性手术治疗，预后较好。其他恶性度较高肾癌的患者在有腔静脉瘤栓的同时也有肾门旁淋巴结肿大患者即使行根治手术治疗，预后仍不良

Abstract:

To know the effect and prognosis of the patients with carcinoma of kidney combining with neoplasia lock in cava. ［Methods］ Seven patients with carcinoma of kidney combining with neoplasia lock in cava accepted the radical operation and were followed up for two years. ［Results］ The carcinoma of kidney and neoplasia lock in cava of the seven patients were totally resected. There were four clear cell carcinomas，the other three were uncommon pathological types. For the patients with clear cell carcinoma，Three patients survived over two years after operation without relapse and metastasis,and one lost follow-up in the cases with clear cellcarcinoma but all of the other three patients with other types of carcinoma died from relapse of tumor in one year after operation. ［Conclusions］ Patients with carcinoma of kidney combining with neoplasia lock in cava still can accept the radical operation. For the patients with clear cell carcinoma combining with neoplasia lock in cava , there is good prognosis. For patients with other types of kedney carcinoma, their prognosis is unsatisfacted other the radical operation is adopted
