T2 mapping与T2* mapping定量值在前列腺病变中的诊断价值

摘要:

目的 探讨前列腺组织良恶性病变的T₂ mapping与T₂^* mapping定量值的差异，为前列腺良恶性病变的诊断与鉴别诊断提供依据。方法 收集50例前列腺病变患者的临床资料，其中前列腺癌19例，良性前列腺病变31例，均具有手术病理或超声引导下穿刺活检病理。通过常规MRI检查及T₂ mapping与T₂^* mapping检查，测量良恶性病灶的T₂ mapping与T₂^* mapping定量值，分析两者之间的差异，绘制ROC曲线，确定最佳诊断临界值及其敏感性和特异性，评价诊断效能。结果 前列腺良恶性病变的T₂ mapping与T₂^* mapping定量值差异具有统计学意义（P<0.01），其中良恶性病变的T₂ mapping与T₂^* mapping定量值分别为（93.15±11.17）ms vs（43.21±15.39）ms和（74.50±6.93）ms vs（21.53±6.81）ms；T₂mapping和T₂^* mapping定量值诊断的敏感性和特异性，最佳截断值，AUC，分别为68.4% vs 94.7%，96.8% vs 74.2%，77.49 ms vs 31.66 ms，0.91 vs 0.88。结论 T₂ mapping与T₂^* mapping定量值有助于辨别前列腺良恶性的病变，是前列腺磁共振检查序列的良好补充。

Abstract:

Objective To investigate the difference between quantitative values of T₂ mapping and T₂^* mapping in benign and malignant lesions of prostate, in order to provide a basis for diagnosis and differential diagnosis of benign and malignant prostate lesions.Methods A total of 50 cases were studied,including 19 cases of prostate cancer and 31 cases of benign prostate lesions,all of which had surgical pathology or ultrasound-guided needle biopsy pathology. Routine MRI,T₂ mapping and T₂^* mapping were performed for all patients.The quantitative values of T₂ mapping and T₂^* mapping of benign and malignant lesions were measured and analyzed.The optimal diagnostic threshold,and its sensitivity and specificity were evaluated through ROC curve. Results There was significant difference between T₂ mapping and T₂^* mapping in benign and malignant prostatic lesions(P<0.01).The quantitative values of T₂ mapping and T₂^* mapping in benign and malignant lesions were(93.15±11.17)ms vs (43.21±15.39)ms and (74.50±6.93)ms vs (21.53±6.81)ms, respectively. The best cut-off values for the quantitative values of T₂mapping and T₂^* mapping, AUC, diagnostic sensitivity and specificity were 77.49 ms vs 31.66 ms, 0.91 vs 0.88, 68.4% vs 94.7% and 96.8% vs 74.2%,respectively. Conclusion The quantitative values of T₂ mapping and T₂^* mappingare helpful to distinguish benign and malignant prostate lesions,and are good supplement to prostate MRI sequence.
