前庭诱发肌源性电位评估帕金森病患者前庭功能的研究

摘要:
目的 利用前庭诱发肌源性电位(vestibular-evoked myogenic potentials,VEMP)检测技术,评估帕金森病(Parkinson's disease,PD)患者前庭丘脑通路的完整性。方法 选择2017年6月至2018年12月就诊于大连市第三人民医院的符合入组条件的PD患者(PD组)60例(按照HOEHN&YAHR量表分级分为早期组30例和中期组30例)及健康人30例(对照组),分别行VEMP检查,记录波幅引出率及各波潜伏期,进行比较分析。结果 PD组与对照组相比,潜伏期延长(或未引出肯定波形),两组差异具有统计学意义(P<0.05)。与早期组相比,中期组波形分化差,潜伏期延长更为明显(P<0.05)。结论 VEMP可以客观反映PD患者前庭丘脑通路的完整性,随着PD的进展前庭丘脑通路功能障碍更明显。

Clinical significance of vestibular myogenic evoked potential in patients with Parkinson's disease

Abstract:
Objective To assess the integrity of the vestibular thalamic pathway in patients with Parkinson's disease by vestibular - evoked myogenic potentials (VEMP) detection technology. Methods Totally 60 patients with Parkinson's disease (PD) were divided into early group and middle group according to the HOEHN&YAHR scale, with 30 patients in each group and 30 healthy individuals in control group. All received VEMP, recording the amplitude extraction rate and latency of each wave for statistical analysis. Results Compared with the healthy control group, the incubation period of PD group was prolonged (or no positive waveform was induced) (P<0.05). Compared with early group, middle group had poor waveform differentiation and more obvious prolonged incubation period (P<0.05). Conclusion VEMP can objectively reflect the integrity of the vestibular thalamus pathway in PD patients. With the progress of PD, dysfunction of the vestibular thalamus pathway becomes more recognizable.
