目的 探讨80kVp低管电压扫描联合全模型迭代重建(IMR)冠状动脉CTA成像的可行性。方法 256层螺旋CT冠状动脉CTA扫描BMI为20~25 kg/m2的患者60例;其中30例患者行80kVp扫描采用IMR重建(A组),30例患者行100kVp扫描采用iDose4算法重建(B组)。采用定量及定性方法分别对两组图像质量进行评价,并对两组的图像质量及辐射剂量进行对比分析,定量图像质量评价指标包括冠脉主要分支近、中、远段CT值、图像噪声及对比噪声比(CNR);定性图像质量评价指标包括血管边缘锐利度、条纹伪影及总体图像质量,采用5分法。结果 80kVp扫描较100kVp有效辐射剂量降低45.3%。A组冠脉各节段CT值及CNR均明显高于B组(P<0.05)。A组冠脉各节段图像噪声均低于B组。A组各定性图像质量指标评分均明显高于B组(P<0.05)。结论 对于BMI为20~25 kg/m2的患者,80kVp低管电压冠状动脉CTA扫描联合IMR可实现低剂量冠状动脉成像。 |

Objective To investigate the feasibility of coronary computed tomographic angiography (CTA) at 80kVp combined with iterative model reconstruction (IMR). Methods Sixty patients with BMI of 20-25 kg/m2 underwent 256-slice coronary CTA. Among them, 30 patients were subjected to 80kVp voltage scanning, followed by algorithm of IMR (group A) and 30 patients were screened with 100kVp voltage scanning combined with iDose4 (group B). Quantitative and qualitative indicators were used to evaluate the image quality of the two groups. The quantitative indicators included CT value, images noise, and compared noise ratio (CNR) of the proximal, middle and distal part of the coronary's main branch. The qualitative indexes included vascular edge sharpness, streak artifacts and image quality, all of which were evaluated by a 5-point system. Results The effective radiation dose of 80kVp decreased by 45.3% compared with 100kVp. The CT value and CNR in each segment of coronary artery in group A were significantly higher than those in group B (P<0.05). The image noise of each segment of coronary artery in group A was lower than that in group B. The qualitative image quality index scores of group A were significantly higher than those of group B (P<0.05). Conclusion For patients with a BMI of 20-25 kg/m2, Low-dose coronary artery imaging can be achieved by 80kVp low-tube voltage coronary CTA combined with IMR. |
