自体骨髓血干细胞不同给药途径治疗糖尿病足的临床疗效观察

摘要:
目的 观察自体骨髓血干细胞不同给药途径治疗糖尿病足的临床疗效。方法 将2008年1月至2015年12月86例膝下缺血的糖尿病足患者按照随机数字法分为股动脉穿刺注射组与肌肉注射组,每组43例。两组在常规治疗的基础上,股动脉穿刺注射组予自体骨髓血干细胞经患侧股动脉穿刺注入,肌肉注射组予自体骨髓血经患肢小腿肌肉穿刺注入。比较两组患者移植前后患足疼痛、皮温及患侧踝肱指数(ABI)的变化。结果 两组患者治疗后患足皮温及疼痛评分均优于治疗前(P<0.05),两组患者组间比较差异无统计学意义(P>0.05),两组患者治疗前后ABI水平比较差异无统计学意义(P>0.05)。结论 自体骨髓血干细胞经股动脉穿刺注入治疗膝下缺血性糖尿病足疗效显著,安全,创伤小,值得临床推广使用。

Abstract:
Objective To observe the clinical effect of autologous bone marrow stem cells on diabetic foot by different administration pathway. Methods From January 2008 to December 2015, 86 patients with diabetic foot and occlusion of inferior genicular artery were randomly divided into femoral artery puncture injection group and intramuscular injection group. In addition to the conventional treatment, autologous bone marrow stem cells were injected into femoral artery in the femoral artery puncture injection group, while the autologous bone marrow blood was injected into lower leg muscle in the intramuscular injection group. The changes before and after transplantation were compared between the two groups,including foot pain, skin temperature and ABI. Results The foot skin temperatures and pain scores after treatment in the two groups were better than those before treatment (P<0.05). No statistically significant difference was observed between the two groups when compared each other (P>0.05). The levels of ABI showed no significant difference before and after treatment in both groups (P>0.05). Conclusion Autologous bone marrow stem cells injection into femoral artery is effective, safe and minimally invasive for the treatment of diabetic foot. It is worthy of clinical promotion.
