缺氧诱导因子-1α、同型半胱氨酸、胱抑素C与2型糖尿病性眼肌麻痹的相关性研究

摘要:
目的 探讨血清缺氧诱导因子-1α(HIF-1α)、同型半胱氨酸(Hcy)、胱抑素C(CysC)在2型糖尿病性眼肌麻痹中的作用及其相关性。方法 采用病例-对照方法研究,根据是否合并糖尿病眼肌麻痹,将120例2型糖尿病患者分为两组,其中2型糖尿病性眼肌麻痹组(A组,60例)、2型糖尿病组(B组,60例),另选同期60例健康体检者为对照组(NC组)。测定受试者血清HIF-1α、Hcy和CysC浓度,记录各组年龄、病程以及相关生化指标。分析各组血清HIF-1α、Hcy和CysC差异及其影响因素。分析糖尿病眼肌麻痹的危险因素。结果 A 组B 组的血清HIF-1α、Hcy、CysC浓度高于NC组,B组的血清HIF-1α浓度高于A组,具有统计学差异,P均< 0.05,A 组血清Hcy、CysC高于B组,P<0.05。HIF-1α与病程、HbA1c、FBG显著负相关,Hcy与病程、HbAlc、FBG、CysC呈正相关,CysC 与病程、Hcy 呈正相关。糖尿病病程≥5年,HbA1c≥7%,Hcy≥10 μmol/L,CysC≥1 mg/L是糖尿病眼肌麻痹的危险因素。结论 HIF-1α、Hcy、CysC与糖尿病性眼肌麻痹相关,检测其血清水平对糖尿病性眼肌麻痹的预防及治疗具有重要的临床意义。

Abstract:
Objective To investigate the levels of HIF-1α, homocysteine(Hcy) and Cystatin C(CysC)in patients with type 2 diabetic ocular muscles palsy. Methods This was a case control study, we divided patients into 2 groups,group A(type 2 diabetic mellitus patients with type 2 diabetic ocular muscles palsy,60),group B(patients with type 2 diabetic mellitus,60),and then the group NC that contains 60 normal controlsas control group. Levels of HIF-1α, Hcy, CysC were measured. Age, duration of disease, blood biochemical indexes were recorded. The correlation analysis of these were performed between three groups, identify the risk factors in patients with type 2 diabetic ocular muscles palsy. Results The serum levels of HIF-1α, Hcy, CysC in group NC were the lowest(P<0.05),The serum level of HIF-1α in group B was higher than group A(P<0.05). The serum levels of Hcy, CysC in group A were higher than group B(P<0.05). The serum level of HIF-1α was negatively correlated with duration of the disease, HbA1c,FBG.The serum level of Hcy was positively correlated with duration of the disease,HbA1c,FBG,CysC.CysC was positively correlated with duration of the disease and Hcy.Duration of the disease≥5 years,HbA1c≥7%,Hcy≥10 μmol/L,CysC≥1 mg/L are significant risk factorsfor type 2 diabetic ocular muscles palsy.Conclusion HIF-1α, Hcy, CysC have correlation with type 2 diabetic ocular muscles palsy. To measure these is significant to the prevention and treatment of type 2 diabetic ocular muscles palsy.
