指动脉背侧支皮瓣与掌背动脉穿支皮瓣对手指软组织缺损的修复效果比较

摘要:

目的　比较指动脉背侧支皮瓣与掌背动脉穿支皮瓣对手指软组织缺损修复的效果。方法　将在大连市第三人民医院就诊的92例手指软组织缺损患根据治疗方式不同分为两组，46例对照组患者行掌背动脉穿支皮瓣治疗，46例观察组患者行指掌侧固有动脉背侧支皮瓣治疗，对比两组患者治疗效果。结果　观察组患者手功能优良率略高于对照组，但组间比较差异无统计学意义（P>0.05）；两组患者两点辨别觉比较差异存在明显统计学意义（P<0.05）；且观察组修复外观良好率显著高于对照组（P<0.05）。结论　指动脉背侧支皮瓣与掌背动脉穿支皮瓣在手指软组织缺损修复中均可发挥较为理想的效果但相对于掌背动脉穿支皮瓣修复法而言，指动脉背侧支皮瓣修复可更好地改善患者指端外观及感觉功能。

Abstract:

ObjectiveTo investigate the result of dorsal branch flap and dorsal metacarpal artery perforator flap in repairing soft tissue defect of the finger.MethodsIn our hospital, 92 cases of soft tissue defect of finger were divided into two groups based on different treatment, 46 cases in the control group treated with dorsal metacarpal artery perforator flap and 46 cases in the observation group treated with volar inherent artery dorsal flap. Therapeutic effect was compared between the two groups of patients.ResultsThe excellent and good rates of hand function in the observation group were slightly higher than those in the control group, but there was no significant difference between the two groups (P>0.05). There was significant difference between the two groups in the two-point tactile discrimination (P<0.05). The observation group had significantly better appearance after repair than the control group (P<0.05).ConclusionArterial dorsal carpal flap and dorsal metacarpal artery perforator flap can play an ideal effect in the repair of soft tissue defect of the finger. Compared with the dorsal metacarpal artery perforator flap, flap repair can better improve the appearance of the finger and sensory function.
