超声牛眼图诊断在不同心脏疾病中的应用

摘要:
超声牛眼图是基于斑点追踪技术建立起来的心脏超声诊断新方法,可以定量分析心肌组织运动的速度、位移、应变、应变率及旋转角度等动力学行为,综合评价心脏整体与局部的收缩和舒张功能变化,直观、迅速、准确地判定心脏不同节段区域心肌受损的严重程度和病变范围,近年受到美国超声心动图学会和中国超声学会新颁心脏定量测量指南的大力推介。本文概括阐述了超声牛眼图的生成原理和模式类型,简要介绍了超声牛眼图的参数采集和分析方法,重点说明了超声牛眼图在常见心脏疾病,如冠心病、高心病、糖尿病心脏病、甲状腺桥本病心脏损害、原发性心肌病、肺心病、心力衰竭以及评价心脏再同步化治疗等的主要表现和诊断价值,并客观分析了二维超声牛眼图存在的不足之处和未来发展走向,以期推动此项新技术应用的进一步开展。

Abstract:
The ultrasonic bull's eye map is a new method, which is established by ultrasonic speckle tracking technology. It can quantitatively analyze cardiac dynamic behaviors such as myocardial tissue velocity, displacement, strain, strain rate and rotation angles, comprehensively evaluate the global and regional cardiac systolic and diastolic functions, and determine lesion severity and range of myocardial dysfunctions in different heart segments directly, rapidly, and accurately. This method has been strongly recommended by the American Echocardiography Society and the new guidelines for cardiac quantitative measurement published by the Chinese Ultrasound Society. This paper reviews the generation principle and pattern classification of ultrasonic bull's eye map, introduces the parameter collection and observation analysis, and focuses on the main manifestations and diagnostic values of ultrasonic bull's eye map in common heart diseases, such as coronary heart disease, hypertensive heart disease, diabetic heart disease, thyroid Hashimoto's heart damage, cardiomyopathy, pulmonary heart disease, heart failure and the cardiac resynchronization therapy. Finally, the objective conclusions of the shortcomings and the development of bull's eye map are also discussed, in order to promote the further utilization of this new technology.
