数字化三维重建在茎突综合征临床诊疗中的应用

摘要:
目的 应用多层螺旋CT(MSCT)检查及其数字化三维重建技术,测量茎突长度、内倾角度、前倾角度,为茎突综合征的诊断和治疗方案提供参考。方法 对55例受试者行临床问卷调查,据咽痛程度用视觉模拟评分法(visual analogue scale,VAS)来进行疼痛评分并分级,同时对受试者进行多层螺旋CT扫描和三维重建,测量出茎突的长度、内倾角度和前倾角度,分析以上三组数据与咽痛程度的关系。同时对受试者分别采取了手术及非手术治疗,分析两组茎突的长度、内倾及前倾角度间的关系。 结果 茎突综合征患者茎突长度及内倾角度与其咽痛的程度呈正相关(P<0.01) ;手术组和非手术组受试者茎突的长度和内倾角度差异有统计学意义(P<0.01)。结论 数字化三维重建技术能够精确测量茎突的长度、内倾及前倾角度,可为茎突综合征的诊断提供较为明确的影像学信息;通过对茎突长度及内倾角度的分析并综合考虑该类患者的临床症状,可以为茎突综合征患者临床诊疗的方案提供参考。

Digitization of three-dimensional reconstruction in diagnosis  and treatment with styloid process syndrome

Abstract:
Objective Using of the digitation of three-dimensional reconstruction of Multi-slice Spiral CT(MSCT) to measure the length, introversion angle, anteversion angle of styloid process, in order to find a correlation with the degree of pharyngeal pain in the styloid process syndrom, then to supply a plan for the diagnose and treatment for styloid process syndrome. Methods We performed a retrospective study on 55 patients(68 sides),used of visual analogue scale(VAS) to grade the pharyngeal pain with the subjects, then according to the grading results to classificate the degree of pharyngeal pain. We measured the length, introversion angle, anteversion angle of the styloid process, and analyzed the relationship among the data, at the same time, we took operation or not-operation treatment, then analyzed the relationship of the length, introversion angle, anteversion angle of the styloid process between two groups. Results In patients of styloid process syndrome, the length, introversion angle of styloid process was in normal phase to the degree of pharyngeal pain(P<0.01); there was statistical significance in the length of the styloid process and the introversion angle of styloid process between the operation and not-operation groups(P<0.01).Conclusion Digitization of three-dimensional reconstruction can measure the length, introversion angle, anteversion angle of styloid process in exact, can provide the imaging information for the styloid process syndrome, according to the analyzing of the data and the clinical symptom of patients, we can provide a plan for diagnose and treatment approach to the styloid process syndrome.
