老年COPD急性加重期患者呼出气NO浓度、PA和SOD水平变化及其临床意义

摘要:
目的 探究老年慢性阻塞性肺疾病(COPD)急性加重期患者呼出气一氧化氮(NO)浓度,血清前白蛋白(PA)和超氧化物歧化酶(SOD)水平变化及其临床意义。 方法 收集2014年5月至2016年2月就诊的老年COPD患者85例,其中急性期患者45例,缓解期患者40例,另选取40例健康体检者为对照组。比较三组NO、PA、SOD水平及肺功能的差异,分析NO、PA和SOD水平与肺功能指标的相关性。 结果 与对照组比较,COPD患者NO、PA和SOD水平及FEV1、FVC、FEV1/FVC%肺功能指标均显著降低(P<0.05);与缓解期患者比较,急性期患者上述指标显著降低(P<0.05)。NO、PA和SOD水平与FEV1/FVC%均呈显著的正相关(r=0.501、0.637和0.562,P<0.05)。 结论 NO、PA和SOD水平与老年COPD患者急性加重密切相关,对监测患者气道炎症及评估COPD病情具有指导意义。

Abstract:
Objective  To explore the levels of exhaled nitric oxide (NO), serum pre albumin (PA) and superoxide dismutase (SOD) in aged patients with acute exacerbation of COPD and the clinical significance. Methods  Totally 85 cases of aged patients with COPD from May 2014 to February 2016 were selected, including 45 cases in acute phase and 40 cases in remission period. Forty healthy persons were used as a control group. Differences of NO, PA, SOD and lung function among the 3 groups were compared, and analysis of relativity between the level of NO, PA, SOD and pulmonary function indexes was performed. Results  Compared with the control group, the levels of NO, PA, SOD and pulmonary function indexes such as FEV1, FVC, FEV1/FVC% in patients with COPD were significantly decreased (P<0.05); Compared with the patients in remission stage, the above indexes were significantly decreased in acute phase (P<0.05). The levels of NO, PA, SOD and FEV1/FVC% showed significant positive correlation (r=0.501, r=0.637, r=0.562, P<0.05). Conclusion  The levels of NO, PA and SOD are closely related to the acute exacerbation of elderly patients with COPD. And the levels of NO, PA and SOD have guiding significance in monitoring the airway inflammation in COPD patients and assessing the severity of the disease.
