初诊外周血小板计数增多对宫颈鳞癌复发生存的影响

摘要:
目的 探讨首次确诊宫颈癌时外周血小板计数增多对宫颈鳞癌患者无复发生存期(RFS)及总生存期(OS)的影响。 方法 回顾分析89例复发性宫颈癌患者的临床资料,观察首次确诊宫颈癌时外周血血小板计数与患者临床病理特征的关系,及对患者RFS和OS的影响。 结果 (1)血小板增多患者中,RFS≤1.5年的患者比例(49.3%)明显高于RFS>1.5年的患者比例(15.0%),P<0.05。(2)肿瘤浸润深度≥0.5 cm和初诊血小板计数≥300×109/L为RFS的独立预后因素(P<0.05)。(3)RFS和血小板计数≥300×109/L为OS的独立预后因素(P<0.05)。 结论 初诊时血小板增多提示宫颈鳞癌转移风险增加,并预示较差的RFS和OS。

Abstract:
Objective To investigate the influence of inereased peripheral blood platelet count at the initial diagnosis of cervical cancer on recurrence-free survival and overall survival of patients with cervical squamous cell carcinoma.  Methods In 89 recurrent cervical cancer patients, we retrospectively analyzed the correlation between peripheral blood platelet count at initial diagnosis and cancer recurrence, as well as the influence of clinicopathological features of recurrent cervical squamous carcinoma on recurrence-free survival (RFS) and overall survival (OS).  Results The increase of peripheral blood platelet count at initial diagnosis was correlated to recurrence free survival (P<0.05). Depth of tumor invasion and platelet count were independent prognostic factors for RFS (P<0.05). RFS and platelet count were independent prognostic factors for OS (P<0.05).  Conclusion The increase in peripheral blood platelet count at initial diagnosis represents a high risk of metastasis in patients with cervical squamous cancer, indicating poor RFS and OS.
