联合检测血清SP-D、hs-CRP及血浆Fbg水平在慢性阻塞性肺疾病患者病情监测中的价值

摘要:
[摘要] 目的 探讨联合检测血清肺泡表面活性蛋白-D(SP-D)、超敏-C反应蛋白(hs-CRP)及血浆纤维蛋白原(Fbg)水平在慢性阻塞性肺疾病患者病情监测中的价值。 方法 选取慢性阻塞性肺疾病急性加重(AECOPD)患者30例(AECOPD组),COPD稳定期患者10例(COPD稳定期组),健康对照30例(健康对照组)。分别检测3组研究对象的血清SP-D、hs-CRP水平,比较其差异,同时检测AECOPD组及COPD稳定期组患者血浆Fbg水平,并比较其差异。分析血清SP-D、hs-CRP及血浆Fbg水平与FEV1%pred、FEV1/FVC的相关性。 结果 (1)血清SP-D水平AECOPD组(21.90±7.89)ng/mL高于健康对照组(8.07±4.03)ng/mL与COPD稳定期组(12.49±3.52)ng/mL(P<0.05),COPD稳定期组高于健康对照组(P<0.05)。(2)血清hs-CRP水平AECOPD组(14.23±7.00)mg/L高于健康对照组(1.06±1.09)mg/L与COPD稳定期组(3.19±3.04)mg/L(P<0.05),COPD稳定期组与健康对照组比较,差异无显著性意义(P>0.05)。(3)血浆Fbg水平AECOPD组(4.27±1.15)g/L高于COPD稳定期组(3.38±1.26)g/L(P<0.05)。(4) COPD患者血清SP-D、hs-CRP及血浆Fbg水平与FEV1%pred、FEV1/FVC无关。 结论 血清SP-D、hs-CRP及血浆Fbg水平可作为COPD诊断及病情监测的良好指标;联合检测上述指标可增加敏感性及特异性。

Abstract:
[Abstract] Objective To discuss the clinical value of combined detection of serum SP-D, hs-CRP and plasma fibrinogen in monitoring patients with chronic obstructive pulmonary disease. Methods Thirty AECOPD patients(AECOPD group),ten patients with COPD at stable phase(COPD stable phase group),thirty healthy people(control group) were selected in this study. And their SP-D,hs-CRP in serum were tested, the differences in three groups were compared. Also plasma fibrinogen in two COPD groups were tested, their differences were compared. Then we analyzed correlations between levels of serum SP-D, serum hs-CRP, plasma fibrinogen and FEV1%pred, FEV1/FVC. Results (1)Serum levels of SP-D in the AECOPD group were higher than those in COPD stable phase group and control group(P<0.05); Serum levels of SP-D in COPD stable phase group were higher than those in control group(P<0.05). (2) Serum levels of hs-CRP in the AECOPD group were higher than those in COPD stable phase group and control group(P<0.05); Comparison between COPD stable phase group and healthy control group about serum levels of hs-CRP had no significant difference(P>0.05). (3) The levels of plasma Fbg in the AECOPD group were higher than those in COPD stable phase group(P<0.05).(4) The correlation analysis showed that levels of serum SP-D, hs-CRP and plasma fibrinogen in two COPD groups were no correlated with FEVl%pred and FEV1/FVC (P>0.05). Conclusions The levels of serum SP-D, hs-CRP and plasma fibrinogen can be as good indicators in the diagnosis and condition monitoring of COPD. Combined detection of these biomarkers can increase the sensitivity and specificity, which also will be significant for improving prognosis.
