经直肠超声引导穿刺治疗前列腺脓肿11例报告

摘要:

[摘要]　目的　探讨前列腺脓肿的临床特点及经直肠超声引导介入治疗的疗效。　方法　回顾性总结2005年2月至2013年6月治疗的11例前列腺脓肿患者的临床资料。分析影像学特点及经直肠超声引导下前列腺脓肿穿刺引流联合抗菌类药物灌洗治疗的效果。　结果　10例前列腺脓腔均一次穿刺成功，抽出脓液10～55 mL。1例首次穿刺后体温未见明显好转，5日后二次穿刺，抽出脓腋约20 mL，甲硝唑灌洗后临床症状明显缓解。所有患者穿刺后1周复查经直肠超声提示脓腔消失。　结论　经直肠超声引导穿刺引流治疗前列腺脓肿，方便，且疗效显著，可作为首选治疗。

Abstract:

[Abstract]　Objective　To evaluate the therapeutic effects of interventional transrectal ultrasound-guided aspiration in the management of prostatic abscess.　Methods　 Between June 2013 and February 2015, 11 cases of prostatic abscess were retrospectively reviewed including clinical history，radiographic characteristics and interventional therapeutic effects by transrectal ultrasound-guided aspiration combined with antimicrobial lavage．After repetitive washing with Metronidazole, 2-5 mL antibiotic were left in the cavity. Results　The procedure was successfully performed in all 10 cases by first attempt. Approximately 10-55 mL purulent material were aspirated. Only 1 patient needed second aspiration due to persistent fever after first procedure. Cavity disappeared in all cases when examined by transrectal ultrasound in 1 week.　Conclusions　Transrectal ultrasound-guided aspiration is convenient and effective, thus can be considered as the first choice in the management of prostatic abscess.
