[摘要] 目的 通过观察阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者血清白细胞介素(IL-18)和巨噬细胞移动抑制因子(MIF)水平,研究二者与OSAHS之间的关系。 方法 收集唐山市协和医院呼吸内科2013年1月至2014年1月经多导睡眠监测仪(PSG)诊断为OSAHS的患者66例,记录其体质量指数(BMI)、呼吸暂停低通气指数(AHI)、最低脉搏血氧饱和度(LSpO2)。根据AHI分为轻中度组(轻度:5次/h≤AHI<15次;中度:15次/h≤AHI<30次/h)共32例,重度组(AHI≥30次/h)34例,另选择年龄、性别及BMI与OSAHS相匹配的同期同院门诊健康体检者25例作为对照组。应用双抗体夹心法对各组血清IL-18及MIF进行检测。 结果 OSAHS患者血清IL-18浓度[轻中度组(15.57±5.09)ng/L和重度组(19.26±9.87)ng/L\]、
MIF浓度[轻中度组(11.47±3.10)μg/L和重度组(14.31±6.28)μg/L\]均高于正常对照组,差异均有显著性意义(P<0.05)。OSAHS组血清IL-18与MIF呈显著正相关。OSAHS组血清IL-18、MIF的水平与AHI呈显著正相关,与睡眠监测中的LSpO2呈显著负相关。 结论 OSAHS患者血清MIF、IL-18水平的变化可反映OSAHS患者病情严重程度及低氧血症严重程度。 |

[Abstract] Objective To investigate serum levels of interleukin-18 (IL-18) and macrophage migration inhibitory factor (MIF) in patients with obstructive sleep apnea hypopnea syndrome(OSAHS) and to explore the relationship between inflammation and OSAHS. Methods According to PSG standards of OSAHS, 66 cases of OSAHS were selected in the Department of Pulmonary Medicine from January 2013 to January 2014. Body mass index (BMI), apnea-hypopnea index (AHI), and minimum pulse oximetry (LSpO2) were measured. According to the AHI, patients were divided into two groups: mild-moderate group (32 cases, mild 5 times/h≤AHI<15 times/h, moderate 15 times/h≤AHI<30 times/h) and severe group (34 cases, AHI≥30 times/h). 25 healthy people, who visited the hospital during the same period, were included as control group with matched age, sex and body mass index (BMI) with OSAHS patients. Serum levels of IL-18 and MIF were measured. Results Serum levels of IL-18 and MIF in OSAHS patients were higher than the control group (P<0.05); the heavier of OSAHS severity, the higher of serum IL-18 and MIF. Serum IL-18 and MIF in OSAHS group were positively correlated. The level of IL-18 and MIF has significant positive correlation with sleep apnea-hypopnea index (AHI) in OSAHS group.The level of IL-18 and MIF has significant negative correlation with LSpO2 in OSAHS group. Conclusion The level of IL-18 and MIF in patients with OSAHS may reflect the severity of the disease and the severity of hypoxemia. |
