相对超声造影参数在肾脏良恶性肿瘤鉴别诊断中的价值

摘要:
[摘要] 目的 分析肾脏良恶性肿瘤超声造影(CEUS)时间-强度曲线(TIC)特点,探讨相对超声造影参数在鉴别肾脏良恶性肿瘤中的临床应用价值。 方法 对大连医科大学附属第一医院的67例经病理证实的肾脏肿瘤行CEUS检查,获取TIC曲线和多个定量参数,包括峰值强度(Peak)、达峰时间(Tp)、曲线下面积(AUC)及造影剂平均通过时间(MTT),比较相对造影参数在肾脏良恶性肿瘤之间的差异。 结果 恶性肿瘤的相对Peak和相对AUC均明显高于良性肿瘤;恶性肿瘤的相对MTT短于良性肿瘤,差异有显著性意义(P<0.05)。相对Tp在良恶性肿瘤之间比较差异无显著性意义(P>0.05)。 结论 超声造影定量参数分析能够提供更多的血流灌注特征,可为肾脏良恶性肿瘤的诊断及鉴别诊断提供重要的参考依据。

Abstract:
[Abstract] Objective To evaluate the application value of relative contrast-enhanced ultrasound (CEUS) parameters in differential diagnosis of renal tumors by analyzing quantitatively the characteristics of time-intensity curve (TIC).  Methods Sixty-seven patients with renal tumors confirmed by surgery and pathology underwent contrast-enhanced ultrasound preoperatively. The time -intensity curves and quantitative parameters were obtained, including peak intensity (Peak), time to peak (Tp), area under curve (AUC) and mean transit time (MTT). The relative CEUS parameters were compared in benign and malignant renal tumors.  Results In malignant tumors, the relative peak intensity and relative area under the curve were more than those of the benign, while the relative mean transit time was less than that of the benign (P<0.05). There was no significant difference in relative time to peak between malignant and benign renal tumors.  Conclusion CEUS can provide more blood flow perfusion characteristics and provide a reference basis for the diagnosis and differential diagnosis of benign and malignant renal tumors, which has important clinical application value.
