胃癌患者血清脂肪酸合酶表达水平的诊断意义

摘要:
[摘要] 目的 探讨胃癌患者血清脂肪酸合酶(FAS)表达水平的诊断意义。 方法 收集 2012 年 1 月—2013 年 12 月在南京医科大学第二附属医院首次诊断为原发性胃癌的患者53例,其中Ⅰ和Ⅱ期患者24例,Ⅲ和Ⅳ期患者29例,并以51例来自健康体检的汉族人群与52例慢性浅表性胃炎的患者为对照,通过ELISA方法对研究对象的血清FAS表达水平进行定量检测。 结果 胃癌患者血清FAS的表达水平(5.25±0.48)ng/mL,明显高于健康对照组(2.82±0.28) ng/mL与慢性浅表性胃炎组(3.24±0.37)ng/mL,P<0.05。通过ROC曲线分析,确定诊断胃癌的血清FAS最佳临界值为3.78 ng/mL。相较于Ⅰ和Ⅱ期患者FAS:(4.04±0.35)ng/mL,Ⅲ和Ⅳ期患者血清FAS(5.83±1.37) ng/mL表达水平更高(P<0.05)。10例患者在手术前血清FAS水平>3.78 ng/mL,其中有7例术后下降至<3.78 ng/mL (P<0.05)。 结论 血清FAS表达水平的升高与胃癌的发生、进展有关,对血清FAS含量的检测有助于胃癌的诊断与随访。

Abstract:
[Abstract] Objective To investigate the diagnostic significance of serum fatty acid synthase(FAS) level in patients with gastric carcinoma. Methods Clinical data of 51 healthy controls,52 chronic superficial gastritis patients and 53 gastric cancer patients, who were firstly diagnosed as gastric carcinoma in the 2nd affiliated hospital of Nanjing Medical University from January 2012 to December 2013, were retrospectively analyzed. Serum FAS level was measured by ELISA and compared across patient groups and healthy controls. Results Significantly higher levels of serum FAS were found in the gastric cancer patients (5.25±0.48)ng/mL compared to the healthy controls (2.82±0.28)ng/mL,P<0.05, and chronic superficial gastritis patients (3.24±0.37) ng/mL,P<0.05. Based on ROC curve analysis, cut off value for gastric carcinoma in the current study was 3.78 ng/mL. Expression levels of serum FAS were higher in the patients with advanced clinical stage [Ⅲ and Ⅳ, (5.83±1.37)ng/mL] compared to those with early stage [Ⅰand Ⅱ, (4.04±0.35)ng/mL P<0.05]. Ten patients had preoperative FAS levels >3.78 ng/mL. The postoperative serum FAS levels decreased to <3.78 ng/mL(P<0.05)in 7 of them. Conclusion These results indicate that serum FAS is a potential biomarker for the detection of gastric cancer.
