摘要:

[摘要] 目的观察腺样体切除手术对儿童慢性鼻窦炎疗效的影响。方法　对112例病程1年以上经保守治疗无明显好转并且合并腺样体肥大的慢性鼻窦炎患儿，行腺样体切除手术，术后随诊1～3个月，观察鼻窦炎治愈情况。结果　在腺样体切除术后3个月内，99例临床症状及体征消失，治愈率为88.4%，13例术后症状体征减轻，好转率为11.6%，有效率100%；所有病例术后鼻阻力曲线均较术前提高，提高幅度为125%～380%；所有病例鼻声反射图像上显示鼻咽部曲线的切迹变小或者消失，双侧鼻腔横截面积较前提高，提高幅度为12%～53%。结论　腺样体切除手术对儿童慢性鼻窦炎的治愈具有积极影响，对于久治不愈的慢性鼻窦炎合并腺样体肥大患儿可优先考虑行腺样体切除手术。

Abstract:

[Abstract] Objective To study the treatment effect of adenoidectomy on chronic sinusitis in children. Methods Totally 112 children with chronic sinusitis were enrolled in the current study. The patient had been managed with conservative treatment for more than one year without significant clinical improvement and was complicated by adenoid hypertrophy. After adenoid resection, the patients were followed up for 1-3 months to observe the treatment effect on sinusitis. Results Clinical symptoms and signs of chronic sinusitis disappeared in 99 cases within 3 months after adenoidectomy with a curative rate of 88.4%. The clinical symptoms and signs relieved in 13 cases after surgery with an improvement rate of 11.6%. The overall effective rate was 100%. After surgery, the nasal resistance curve improved by 125%-380% in all of the cases. Nasal acoustic reflection image showed the notch of nasopharyngeal curve became smaller or disappeared in all the cases. Bilateral nasal cross-sectional area increased 12%-53%. Conclusion Adenoidectomy has positive effects on children with chronic sinusitis. Children suffering from persistent chronic sinusitis complicated with adenoid hypertrophy are given the priority for adenoidectomy.
