目的 探讨前列腺体积及前列腺突入膀胱程度与最大尿流率的关系。 方法 对141例诊断为前列腺增生症的患者首先采用经直肠超声测量前列腺体积 (PV) 及前列腺突入膀胱程度(IPP),然后使用尿动力学检测仪检测最大尿流率,并且按照最大尿流率的大小分为A组(>15 mL/s)、B组(10~15 mL/s)、C组(≤10 mL/s)。分别比较3组间PV值和IPP值的差异,以及对PV、IPP与最大尿流率做回归分析。 结果 3组PV分别为46.627 cm 3、55.162 cm 3、57.078 cm 3,A组与B组、C组间的PV值比较差异有显著性意义 ,B组与C组的体积比较差异无显著性意义。A组的IPP值为(0.0781±1.1669)cm,95%的可信区间0.309~1.252;B组IPP值为(1.879±0.6313)cm ,95%可信区间1.675~2.084;C组的IPP值为(4.703±1.4643)cm ,95%可信区间4.368~5.037。3组IPP值比较结果显示任意两组比较差异有显著性意义(P<0.05)。当判断是否存在可疑膀胱出口梗阻时,PV、IPP都具有统计学意义(P<0.05),而判断是否存在膀胱出口梗阻时,仅IPP具有统计学意义(P<0.05)。 结论 PV>60.4 cm 3或IPP>2.1 cm的患者易发生膀胱出口梗阻,而IPP>5.0 cm的患者发生膀胱出口梗阻的可能性将大大增加;PV、IPP均可作为可疑膀胱出口梗阻的参考指标,而仅IPP可作为膀胱出口梗阻的参考指标。 |

Objective To investigate the relationship between Prostate volume (PV), the degree of intravesical prostatic of protrusion (IPP) and maximal urinary flow rates. Methods The prostate volume (PV) and intravesical prostatic of protrusion (IPP) of 141 benign prostatic hyperplasia (BPH) patients were measured by trans-rectal ultrasound (TRUS), and maximum urinary flow rate of those patients were determined by Urinary dynamics detector at the same time.According to the size of maximum urinary flow rate, 141 benign prostatic hyperplasia (BPH) patients were classified as group A (>15 mL/s), group B (10-15 mL/s) and group C (≤10 mL/s). and then we compared the clinical data of these three groups. Results According to the average value of three groups of PV were 46.627 cm3, 55.162 cm3, 57.078 cm3, there was a statistical significant difference in the PV between group A and group B, group A and group C ,but there was no significant difference in the PV between group B and group C. The degree of IPP had a statistical significance difference between these three groups (P<0.05). The IPP value of group A was (0.0781±1.1669)cm, and 95% confidence interval (CI) was 0.309-1.252. The IPP value of group B was (1.879±0.6313)cm, and 95% confidence interval (CI) was (1.675-2.084); The IPP value of group C was (4.703±1.4643)cm, and 95% confidence interval (CI) was 4.368-5.037. The comparison results in any of the two groups of IPP value had significant difference (P< 0.05) .When determining whether there is suspicious bladder outlet obstruction, PV and IPP have statistical significance (P<0.05). However, when determining whether there is a bladder outlet obstruction, only IPP has statistical significance (P< 0.05). Conclusion There have possibility of occurrence of bladder outlet obstruction when PV>60.4 cm3 or the IPP value > 2.1 cm, and the possibility of occurrence of bladder outlet obstruction will increase when the IPP value > 5.0 cm. |
