血清睾酮及体重指数与前列腺癌关系的研究

摘要:
[目的] 分析血清睾酮(serum testosterone,T)水平、体重指数(body mass index,BMI)与前列腺穿刺结果及前列腺癌Gleason评分之间的关系,探索它们之间可能存在的内在联系。[方法] 筛选出2008年8月—2012年3月间行前列腺13针穿刺并符合本研究标准的患者共56例。根据患者的体重和身高计算出体重指数、血清睾酮与体重指数的比值(T/BMI)。根据病理检查结果将研究对象分为非前列腺癌和前列腺癌两组,再根据Gleason评分将前列腺癌患者分为两组(Gleason≤7组和Gleason≥7组)。分析T、BMI及T/BMI与前列腺穿刺结果、前列腺癌Gleason评分之间的相关性。[结果] 56例患者的年龄与血清睾酮水平之间无显著相关性(r=-0.162, P=0.232)。前列腺癌和非前列腺癌两组间,前列腺癌组的血清睾酮水平较低而体重指数较高(P<0.05),两组间T/BMI值比较差异有显著性意义(P<0.05)。将26例前列腺癌患者按Gleason评分分组,Gleason≥7组的患者血清睾酮水平较低(P<0.05),两组间的体重指数与T/BMI比较差异无显著意义(P>0.05)。[结论] T、BMI和T/BMI可与血清PSA水平、影像学检查、直肠指诊等检查结果结合起来,为诊断前列腺癌、判断预后及指导治疗提供依据。

Abstract:
[Objective] To investigate the association between serum testosterone, body mass index and prostate cancer. [Methods] From August, 2008 to March, 2012, 56 cases who firstly underwent prostate needle biopsy in the Second Hospital of Dalian Medical University. The 56 cases were stratified into two groups by pathological results. The prostate cancer patients were divided into two groups by Gleason score. Variables analyzed included serum testosterone (T), body mass index (BMI), T/BMI, pathological results, and Gleason score. [Results] There was no significant correlation between serum testosterone and age (r=-0.162, P=0.232). The serum testosterone levels of cancer patients were lower than non-cancer patients' and the BMI of cancer patients was higher than non- cancer patients' (P<0.05). The difference of T/BMI between cancer patients and non cancer patients is statistically significant (P<0.05). In prostate cancer patients, the people who had higher Gleason score had lower serum testosterone level (P<0.05), but BMI and T/BMI were not significant different (P>0.05) between high Gleason score group and low Gleason score group. [Conclusion] In those who underwent prostate needle biopsy, lower testosterone level associates with higher incidence of prostate cancer and higher Gleason score, BMI was positive correlation with the incidence of prostatic cancer.
