慢性心力衰竭患者和肽素、尿酸水平变化及其临床意义

摘要:
[目的] 探讨慢性心力衰竭(chronic heart failure,CHF)患者血浆和肽素(copeptin)、血清尿酸水平与心功能变化及左室舒张末期内径(LVEDd)之间的关系和临床意义。[方法] 选取2010年6月—2010年12月于大连医科大学附属第一医院诊断为慢性心力衰竭患者74例(CHF组)和同期健康体检的健康者39例(正常组),比较两组和肽素、尿酸水平。[结果] (1)CHF组血浆和肽素水平及血清尿酸水平明显高于对照组(P<0.01)。血浆和肽素水平在各级心功能组差异无显著性意义。血清尿酸水平随心功能分级逐级升高,差异有显著性意义。心功能Ⅳ级组尿酸水平明显高于Ⅲ级组,Ⅲ级组明显高于Ⅱ级组。 (2) 和肽素水平及尿酸浓度的升高与LVEDd的大小呈正相关(r=0.43及0.46,P<0.05)。[结论] 和肽素和尿酸水平可作为判断CHF患者心功能严重程度、预后的重要指标,两者的联合检测有助于对心功能进行综合评价。

Clinical significance and the expression of copeptin and uric acid in patients with chronic heart failure

Abstract:
[Objective] To detect the copeptin and uric acid levels in patients with chronic heart failure, and to evaluate the difference of the levels in patients with the different LV diameter (LVEDd). [Methods] Seventy-four patients with chronic heart failure as the experimental group, thirty-nine people without organic disease as the control group. The copeptin and uric acid levels were estimated in the experimental group. [Results] (1) The levels of plasma copeptin and serum acid uric in the experimental group were significantly higher than that in control group (P<0.01). The plasma copeptin levels were not increased with the severity of cardiac function classification. The serum acid uric levels were significantly increased with the severity of cardiac function classification. (2) The levels of plasma copeptin and serum acid uric were positively correlated with LVEDd (r=0.43 and 0.46, P<0.05). [Conclusion] The higher levels of plasma copeptin and serum acid uric suggest the wider of LVEDd and the serious standard of CHF.
