[目的]探讨妊娠期高血压疾病高风险孕妇摄取卵磷脂对其血浆前列环素(PGI2)代谢产物(6-keto-PGF1α),血栓素(TXA2)代谢产物(TXB2)水平的影响及改善妊娠结局的可能性。
[方法]选取既往无高血压、无心、肝、肾疾病单胎孕妇于妊娠20~22周应用SD-Ⅱ型妊娠期高血压预测分析系统进行预测,检测出妊娠期高血压疾病高风险(阳性)204例,从中随机选取102例作为实验组,余102例作为对照组,实验组每日口服卵磷脂2 g ,两组均进行饮食调节,运动干预,两组分别于实验前(妊娠20~22周)、实验后(妊娠36~40周)测定血浆前列环素、血栓素水平,统计并比较两组妊娠期高血压、子痫前期、子痫、胎盘早剥发生率等妊娠结局。[结果]妊娠36~40周,实验组TXB2浓度明显降低,6-Keto-PGF1α浓度明显升高,与对照组相比差异有显著性意义
(P<0.05);实验组TXB2/6-keto-PGF1α明显低于对照组,差异有显著性意义(P<0.05 )。实验组妊娠期高血压、子痫前期、子痫、胎盘早剥发生率低于对照组,差异有显著性意义(P<0.05)。[结论]妊娠期高血压疾病高风险孕妇者摄入卵磷脂,对降低妊娠期高血压疾病发病率,改善母儿结局有一定作用。 |

Abstract: |
[Objective]To study the effect of soybean lecithin on the concentration of 6-keto-PGF1α and TXB2 in pregnancy SD-Ⅱ predicting positive HDIP and then improve the outcome of both mother and fetus. [Methods]Two hundred and four pregnant women who had only one fetus, and without hypertension and disease in heart, liver and kidney, were analyzed with SD-Ⅱ predicting HDIP positive at the 20th~22nd week of their pregnancy. They were divided into 2 groups equally. The experimental group took 2 g soybean lecithin every day until the 36th~40th week, Their 6-keto-PGF1α, TXB2, levels were measured. We calculated and compared the incidents of gestational hypertension, preeclampsia, eclampsia and placental abruption in two groups. [Results]At the 36th~40th week, The concentration of TXB2 in experimental group was significantly lower than that of control group (P<0.05). The concentration of 6-keto-PGF1α in experimental group was significantly higher than that of control group (P<0.05). The rate of TXB2/6-keto-PGF1α in experimental group was significantly lower than that of control group (P<0.05). [Conclution]Uptaking soybean lecithin may reduce the incident of HDIP and improve the outcome of both mother and fetus obviously. |
