摘要:

［目的］探讨胆总管切开取石术的优点、方法和适应证。［方法］22例胆总管结石患者行腹腔镜胆总管切开取石术。［结果］22例手术均获成功，无中转开腹，其中T管引流17例，Ⅰ期缝合5例，手术时间90～180 min，平均125 min，术中出血20～50 mL，平均35 mL，3例术后胆漏，保守治愈，5例胆道残余结石，术后胆道镜取出。［结论］腹腔镜胆总管切开取石术安全有效，对符合适应症的患者是最佳的治疗方式。

Abstract:

［Objective］To explore the value,method and indication of laparoscopic common bile duct exploration.［Methods］A total of 22 cases with common bile duct stones were treated with laparoscopic common bile duct exploration.［Results］The operation was performed successfully in all of the 22 cases,without conversions to open surgery,17 cases with T-tube drainage,5 cases with primary ductal submucosa closure.The operative time was 90 to 180 min (mean 125 min).The blood loss was 20 to 50 mL(mean 35 mL).Bile leakage occurred in 3 cases and was cured with conservative treatment.Residual stones were found in 5 cases and a postoperative choledochoscopy was required.［Conclusions］Laparoscopic common bile duct exploration is a safe and effective operation,which might be a best option for patients who have the indication.
