[目的] 探讨食管癌组织中上皮钙黏素E-cadherin蛋白的表达特点及意义。[方法] 通过高通量组织芯片技术,制作石蜡组织微阵列,运用免疫组织化学染色ABC方法,检测E-cadherin在83例食管癌组织、38例癌前病变、9例非癌黏膜中的表达情况和分布特点。[结果] 功能性膜E-cadherin蛋白在癌旁、癌前及食管癌组织中呈现逐渐下调的表达趋势,下调率分别为44.4%、84.2%和94.0%,癌旁与癌前、癌组织相比,差异有统计学意义(P<0.05);膜E-cadherin表达未见显著性别和年龄差异(P>0.05),且在不同分化的比较中,差异无统计学意义(P>0.05);但在不同大、中、小病变中及肿瘤TMN分期中差异有显著性意义(P<0.05)。[结论] E-cadherin基因表达是食管癌发生、发展、演进过程中重要的分子事件,其表达下调与食管癌进展密切相关,可以作为判断食管癌病变程度以及预测肿瘤浸润趋势的潜在的分子指标,具有重要的肿瘤诊断学意义。 |

[Objective] The current study aimed to profile expression of E-cadherin during stepwise esophageal carcinogenesis and to analyze the relationship to the series of pathological factors.[Methods] Paraffin embedded tissue array immuno-histo-chemistry methods were adopted to exam the expression pattern of E-cadherin in 83 patients with EC,38 cases with precancerous lesions and 9 cases with relative normal esophageal tissue. The correlation between E-cadherin expression, EC invasiveness and metastasis was analyzed.[Results] The diminished rate of memberous E-cadherin in noncancerous tissues, premalignant tissues and cancer samples were 44.4%, 84.2% and 94.0% respectively. There were significant differences of E-cadherin downregulation both between the EC group and noncancerous mucosa and between noncancerous and premalignant epithelial tissue (P<0.05). No statistical differences were observed in different age, gender, and differentiation level except for tumor size (P>0.05) and statistical significance could be established between different EC TNM staging (P<0.05).[Conclusions] The expression of E-cadherin gene is an important molecular event throughout the whole process of the genesis,development and dissemination of esophageal tumor. E-cadherin decreased expression in different EC TNM staging contributes to invasion and progression of EC, and is of important value in molecular diagnosis and prognosis prediction. |
