[目的] 探讨脑海绵状血管瘤的磁敏感加权成像(SWI)的影像学表现特征及其诊断价值。[方法] 经手术、DSA和/或MRI(MRA、SWI等)检查确诊的脑海绵状血管瘤60例。所有病例均行T1WI、T2WI及SWI序列扫描,其中28例行T1WI增强扫描。应用配对秩和及 t 检验对病灶检出数目及显示病灶范围进行比较。[结果] 本组病例单发40例(66.7%),多发20例(33.3%);SWI发现病灶316个,明显多于T1WI显示的64个(20.3%)和T2WI显示的77个(24.4%)(P<0.05);典型表现爆米花样或桑椹状混杂高信号,边缘呈低信号铁环征或高信号光环征(53.8%)。SWI与T2WI显示病灶面积分别为(3.39±5.68)cm2与(2.40±5.07)cm2(P<0.05)。[结论] SWI较常规MRI序列可检出更多微小病灶,在脑海绵状血管瘤的定性及定量诊断方面极具临床应用价值。 |

[Objective] To investigate the image-features and clinical diagnostic value of cerebral cavernous hemangioma by SWI(Susceptibility Weighted Imaging). [Methods] There were 60 patients with cerebral cavernous hemangioma who were confirmed by operation, DSA and/or MR (MRA, SWI et al) in the group. All of them were scanned by MR which included SET1WI, T2WI, and axis SWI, and 28 cases were additionally scanned with contrast enhancement scan. Paired rank sum test and t-test were used to detect the number and area of lesions. [Results] There were 40 single-cases (66.7%) and 20 multiple-cases(33.3%)in the group. And 316 focuses were detected by SWI, more than that detected by T1WI (64 focuses) and T2WI (77 focuses). It typically showed the sample of popcorn or mulberry-like mixed hypo- and hyperintense with hoop-like hypointensity and halo-like hyperintense on the edge of infection. The areas of lesions are (3.39±5.68)cm2 in SWI and (2.40±5.07) cm2 in T2WI(P<0.05). [Conclusion] SWI can detect slimmer lesions than that is examined by routine-MR scan. So it has important clinical value in qualitative and quantitative diagnoses of the cerebral cavernous hemangioma. |
