应用ROC曲线分析SCC-Ag在宫颈鳞癌中的临床价值

摘要:
[目的] 应用ROC曲线探讨鳞状细胞癌抗原(squamous cell carcinoma antigen, SCC-Ag)在宫颈鳞癌的诊断意义。 [方法] 对31例宫颈鳞状细胞癌患者、33例非宫颈鳞状细胞癌患者、30例正常健康的妇女,采用ELISA方法检测血清中SCC-Ag的含量。建立ROC曲线评价系统。 [结果] 宫颈鳞癌组SCC-Ag测得值的中位数为2.80 μg/L,非宫颈鳞癌组SCC-Ag测得值的中位数为0.51 μg/L,正常对照组SCC-Ag测得值均数为0.34 μg/L。宫颈鳞癌组与非宫颈鳞癌组、正常对照组比较,差异具有统计学意义(P<0.05)。应用ROC曲线分析,曲线下面积为0.853,95%可信区间为0.755~0.950。最佳临界值为1.06 μg/L ,其灵敏度为74.2%,特异度为90.3%。 [结论] 鳞状细胞癌抗原是宫颈鳞癌较特异的肿瘤标志物,可作为宫颈鳞癌的辅助诊断指标。

Abstract:
[Objective] This study is to investigate the diagnostic value of SCC-Ag as a tumor marker in the cervical squamous cell carcinoma using ROC curve analysis. [Methods] SCC-Ag was examined by ELISA in 31 patients with cervical squamous cell carcinoma, 33 patients with non-cervical squamous cell carcinoma, and 30 healthy women. ROC curve was established based on the examination. [Results] The median of SCC-Ag level in the cervical squamous cell carcinoma was 2.80 μg/L, which was higher (P<0.05) than that in non-cervical squamous cell carcinoma (0.51 μg/L) and control groups (0.34 μg/L). According to ROC curve, the area under the ROC curve (AUC) was 0.853 and the 95% confidence interval of the area under the curve was 0.755 to 0.950. The cut off value of SCC-Ag was 1.06 μg/L. The sensitivity was 74.2% and the specificity was 90.3%. [Conclusion] ROC curve analysis shows that SCC-Ag is excellent tumor marker that is very useful for the diagnosis of the cervical squamous cell carcinoma.
