[目的]了解慢性阻塞性肺疾病(COPD)合并肺心病急性加重期患者红细胞、单个核细胞及多形核粒细胞内钾(K)、钠(Na)含量的变化。[方法]选取住院COPD合并肺心病患者40例,健康对照组36例。采用密度-梯度法分离血细胞,以原子吸收分光光度计测定细胞的K、Na含量。
[结果]患者组红细胞K(65.97±12.80)mmol/L、Na(7.93±1.33)mmol/L明显降低(P<0.05),单个核粒细胞K(11.93±3.34)μmol/109cells、Na(27.32±18.76)μmol/109cells无明显改变(P>0.05),多形核粒细胞K(27.32±18.76)μmol/109cells、Na(15.84±12.53)μmol/109cells,高于对照组,差异有显著性意义(P<0.05)。单个核细胞K与多形核粒细胞K、Na及PaCO2呈正相关(相关系数分别为0.567,0.34,0.31,P<0.05)。[结论]COPD合并肺心病患者存在体K的缺失,多形核粒细胞及单个核粒细胞并不能很好地反映体内K、Na的慢性改变。 |

[Objective]To investigate potassium and sodium content of red blood cells, mononuclear and polynuclear blood cells in COPD patients with chronic cor pulmonale.[Methods]Mononuclear and polynuclear blood cells was separated by density-gradient centrifugation. Potassium and sodium content was analyzed by atomic absorption spectrometer.[Results]Potassium (65.97±12.80) mmol/L and sodium (7.93±1.33) mmol/L content of red blood cells decreased significantly in patients (P<0.05), while potassium(11.93±3.34) μmol/109cells and sodium (27.32±18.76) μmol/109cells in mononuclear blood cells remained unchanged (P>0.05). Potassium ( 27.32±18.76) μmol/109cells and sodium (15.84±12.53)μmol/109cells content in polynuclear blood cells increased significantly (P<0.05). Potassium content in mononuclear blood cells has a positive correlation with potassium and sodium content of polynuclear blood cells and PaCO2 (correlation coefficient was 0.567,0.34,and 0.31,respectively. P<0.05)[Conclusions]There is potassium deficiency in COPD patients with chronic cor pulmonale. Potassium and sodium in mononuclear and polynuclear blood cells may not be a good index of intracellular potassium and sodium. |
