血红蛋白含量的变化对腺样体及/或扁桃体切除术失血情况的评价

摘要:
[目的]通过观察儿童全身麻醉或表面麻醉下行腺样体及/或扁桃体切除术后血红蛋白含量的变化,评价患儿失血情况。[方法]35例扁桃体及/或腺样体肥大患儿分别全身麻醉或表面麻醉下行扁桃体及/或腺样体切除术,根据所施手术及麻醉情况分四组,记录手术前后血红蛋白(Hb)含量的变化值。[结果]所有全麻患者术后Hb含量均降低,其中腺样体切除组Hb的降低至0.1~1.8 g/L,扁桃体切除组为0.1~0.4 g/L,扁桃体及腺样体均切除者为0.3~1.8 g/L。表面麻醉下行扁桃体及腺样体切除者Hb的降低在0~0.3 g/L 。[结论]全身麻醉下行腺样体或扁桃体切除术,患儿痛苦小,病灶去除彻底,但需要注意患儿失血增多,必要时检测Hb含量的变化情况可提示医师如何治疗术后出血的患儿。

Abstract:
[Objective]To estimate the blood losing after conducting tonsillectomy and/or adenotomy in children.[Methods]Thirty-five cases suffering from hypertrophy of tonsil and/or adenoid vegetation were divided into 4 groups.They were conducted tonsillectomy or/and adenotomy under the condition of general anesthesia or local anesthesia respectively.The haemoglobin content was measured before and after operation.[Results]For those conducted general anesthesia, the decrease of haemoglobin contents in the group which was underwent tonsillectomy is range from 0.1 to 0.4 g/L,the group underwent adenotomy is 0.1~1.8 g/L,and in the group underwent both tonsillectomy and adenotomy is 0.3~1.8 g/L. The decrease of haemoglobin contents in the group underwent both tonsillectomy and adenotomy but under the condition of local anesthesia is 0~0.3 g/L.[Conclusions]General anesthesia is beneficial to both the children and the doctors when conducting tonsillectomy and/or adenotomy,but more attention should be paid to the children during and after operation becaude they are more likely to lose blood.
