心导纳微分环对冠心病高血压患者的心功能分析

摘要:
探讨冠心病、高血压患者心导纳微分环 (CADL)的环体面积变化、环形改变的特点及差异,进而评价其左心功能,为配合临床诊断 与治疗提供参考。[方法]用心导纳微分环(CADL)对30例正常人、30例冠心病、30例高血 压患者作左心功能检测。从检测结果中选取CADL的总环面积、I相面积、III相面积、V相面 积及I相离心支的改变程度5项指标,比较2个患者组与正常组以及2个患者组之间的差异。 [结果]冠心病、高血压组与正常组比较CADL的总环面积、I相面积均较正常组明显减小,I II、V相面积较正常组亦明显增大,有显著差异(P<0.01);另外,2个患者组中CADL环 的I相离心支都程度不同的出现切迹或波浪状改变。这些改变与正常组的CADL环形有明显不 同,在两个患者组间则无明显差异(P>0.05)。[结论]CADL的环体面积及环形变化可反 映左心功能的变化,其敏感度高于心电图。从左心功能受损程度上看以冠心病组最为严重, 高血压组次之

Abstract:
Discuss about characteristics and differences o f area and shape variability of CADL between the individuals who suffer from coronary hear t disease and individuals suffering from hypertension and estimate for their lef t cardiac function .[Methods] Make use of CADL to detect left cardiac function o f 30 normal persons and 60 patients (30 cases of coronary heart disease and 30 c ases of hypertension). From the results ,five targets about shape variability i n centrifugal branches of phase I and area variability of total loop ,phase I ,ph ase III and phase V are selected to contrast the difference between two groups o f the patients and the group of the normal persons ,and the difference among two patient's groups. [Results] Compared to the normal group ,two patient's grou ps indicate that CADL's total loop area and the area of phase I decrease significa ntly; the area of phase III and phase V increase remarkably .Furthermore , waved changes with various degrees appear at the centrifugal branches of phase I. The se change shows difference (P<0.01) with the normal groups ,while there is n o significant difference (P>0.05)among two groups of patients.[Conclusion ] Changes o f left cardiac function can be indicated by the variability of area and shapes o f CADL, whose sensitivity is higher than ECG's. Regarding the damage extend of l eft cardiac function, the group of coronary heart disease is more serious than t he group of hypertension.
