肝硬化患者血浆内皮素和内毒素水平及相互关系

摘要:
了解不同阶段肝硬化患者血浆的内皮素(ET)及内毒素水平,并探讨它们对肝硬化腹水形成和肝功能损害所起的作用及其相互关系。方法:用放射免疫法测定血浆ET-1浓度,用鲎试验的显色基质法测定内毒素浓度,同时也测定了解肝功能。结果:肝硬化各组血浆内毒素浓度均明显高于正常对照组,有腹水组明显高于无腹水组,肾衰组明显高于无肾衰有腹水组。血浆ET浓度在肝硬化各组均明显高于正常对照组,有腹水组与无腹水组无显著差异,肾衰组明显高于无肾衰有腹水组。 血浆ET-1、内毒素水平随肝功能损害加重逐渐升高,二者水平呈正相关。 结论:血浆内毒素和ET升高可能在肝硬化病程进展中起了重要作用,是引起腹水形成和肾功能损害的重要因素。

Abstract:
To measure plasma endothelin-1 and endotoxin levels in cirrhotic patients with different phrases,and evaluate their roles in ascite production, liver dysfunctions, and analyzed relationships among them. Methods:The plasma endothelin-1 concentration was measured with radioimmunoassay, and endotoxin level was detected with limulus-based chromogenic assay. The liver function were also measured with common meththods. Results: The plasma levels of endotoxin were significantly higher in the three cirrhotic groups respectly than that in healthy subjects, higher in cirrhotic patients with ascites than those without ascites, higher in cirrhotic patients with functional renal failure than those with ascites but without functional renal failure. The plasma concentration of endothelin-1 was significantly higher in the three cirrhotic groups, respectly than that in healthy subjects ,higher in patients with functional renal failure than that without . Plasma endothelin-1 and endotoxin levels increased gradually in relation to the severity of liver dysfunction. In the patients with cirrhosis, there were positive correlations between plasma endotoxin level and concentration of endothelin-1. Conclusions: Increased plasma endothelin and endotoxin may play important roles in cirrhosis procession, may be important factors in ascite production, liver dysfunctions and renal failure.
